To accomplish our mission, CommonHealth is seeking a highly qualified and passionate case manager to help guide our brand and mission in the recovery community. This position is a part-time role with a time commitment of 25-30 hours per week and the possibility of developing into a full time 40 hour per week position.
Your Job as a Targeted Case Manager:
The Targeted Case Manager will provide case management, assessment, and treatment planning services to patients with a substance use disorder and co-occurring mental health disorders. Case Management services mean those activities provided to assist and support individuals in gaining access to needed medical, social, educational, and other services essential to meeting basic human needs. Services may include interactions with family members, other individuals or entities, and includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services and resources. Targeted Case Management services at CommonHealth Recovery consist of (1) assessment of the eligible patient to determine service needs; (2) development of an individualized integrated action plan; (3) referral and related activities to help the patient obtain needed services; (4) monitoring and follow-up; and (5) evaluation.
Primary Responsibilities include:
- Connect patients with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of the referral
- Complete detailed reports on needs assessments and Targeted Case Management activities
- Input accurate and complete Targeted Case Management data into the electronic health record database
- Ensure that all documents submitted on behalf of a patient are valid and authentic
- Assist patients in accessing the appropriate medical, social, educational, and employment resources
- Connect patients and families with support groups and additional behavioral health resources
- Assist patients in securing temporary and long-term housing and shelter
- Schedule treatment appointments for patients and explain the treatment timeline and objectives
- Ensure that patients have reliable and consistent transportation to CommonHealth Recovery and other care providers
- Provide crisis interventions and de-escalation techniques
- Monitor and ensure that patients are seeing other health care professionals outside of CommonHealth Recovery when required by CommonHealth Recovery treatment team
- Assure that documentation is completed in compliance with regulatory agency requirements and best clinical practices
- Coordinate patient care with other CommonHealth Recovery team members including counselors, physicians, and/or psychiatrists
- Adhere to CommonHealth Recovery policies and procedures, and professional code of ethics
- Complete documentation utilizing the agency's designated Electronic Medical Record, within the specified timeframe
- Attend required meetings including but not limited to, individual and group supervision, treatment team meeting, and quarterly in-service; and
- May provide various other services as needed including but not limited to group orientation and counseling.

Targeted Case Manager employed by an outpatient or residential AODE shall have the following requirements:
Required Education/Degree:
Have a bachelor's degree from an accredited college or university; or be a CADC as defined by KRS 309.080(2)
Required Training:
Successfully complete a department-approved Targeted Case Management training within six (6) months of employment as a case manager; and successfully complete ten (10) hours of continuing education in alcohol and other drug-relevant training annually.
