Personalized Health Partners, the medical practice associated with CommuniCare Health Services, is currently recruiting for the key leadership position of Director of Network Program Development and Integration.

Salary Range: $75k-85k

Role and Responsibilities:

The primary role of the Director of Network Program Development is the implementation and ongoing maintenance for success of chronic care management and remote patient monitoring across CommuniCare facilities. In addition, this role will be responsible for oversight and throughput optimizations related to risk or value-based contract arrangements and other clinical programs that Personalized Health Partners participates in.

The responsibilities of the director in this role include, but are not limited to:

• Support work flow creation, implementation, and onboarding for CCM & RPM.
• Support and assist in initiatives for ongoing development to advance the CCM & RPM programs based on growth, as well as future state trends and opportunities.
• Provide program analytics & operational oversight for guidance of strategic initiatives and action planning to achieve continued success for members associated.
• Partner with leadership to develop, disseminate, and use data to guide and support improvement of programmatic performance & enrollment outcomes.
• Ensure regulatory performance team to develop & maintain dashboards that meet DMAS, CMS, NCQA reporting requirements.
• Provide direct oversight & coaching to key communication partners needed across the organization, particularly the direct reporting of the Network & Program Communication Manager.
• Partner with VP, Rev Cycle, and 1bios care team to ensure process and program efficiencies in place to drive member outcomes & PHP provider engagement.
• Monitor and track program metrics, program dashboards, and other reporting tools to drive action and improvements.
• Ensure enrollment follow up and Identify gaps in reporting to ensure growth for the ACO.
• Other duties as assigned with existing or new program participation.

Qualifications:

• Bachelor’s Degree preferred
• 3-5 years’ experience in a managed care organization or project management environment
• 3 years leadership/management experience
• Experience with Medicaid and Medicare populations preferred
