Description

The Integrated Behavioral Health Services Manager is responsible for the coordination and development of Integrated Behavioral Health Services offered at CommuniCare Health Centers. CommuniCare Health Centers is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, mental health and outreach services to low income, multi-ethnic, uninsured and underinsured populations.

 

SCHEDULE:                  40 hours/week which may include evening hours; all sites, evenings may be required.

 

RESPONSIBILITIES:

  1. The Integrated Behavioral Health (IBH) service operates in primary care-based settings and is guided by the larger structure of the Behavioral Health Department. The service model includes BH and mental health/ACEs screenings; acute needs determination and triage; health/social services patient navigation assistance (i.e., care coordination), collaboration with medical providers; brief psychological assessments; and time-limited counseling.
  2. The IBH Manager supervises the service’s treatment staff and care coordinators across multiple sites, contributes as a direct service provider themselves, ensures service/staff compliance with existing protocols and operations standards, exercises responsible stewardship of resources to maximize service reach and outcomes, and seeks continuous improvement in the quality and efficiency of services.
  3. As necessary, duties may involve interaction with emergency or psychiatric services, County, police, other community agencies, and various case management and ancillary staff.
  4. Attend clinical care meetings, clinic managers’ meetings and maintain ongoing relationship with other departments within CCHC and represent integrated care services internally and externally with community partners.
  5. Maintain responsibility for recruiting, training, developing and evaluating staff (including clinicians, case managers, navigators an interns etc.)
  6. Participate in and directly observe clinical and case management delivery of services through regular and frequent interface with program staff.
  7. Provide regular and timely feedback and reports to medical and behavioral health administrative leadership and other stakeholders pertaining to IBH service operations, as requested or necessary. In these efforts, identify program achievements as well as barriers, suggestions for improvement, and new service opportunities.
  8. Maintain and evaluate the need for improvements to IBH workflows and program support documentation, e.g., EHR and data collection workflows, staff onboarding checklists, program manuals, workflow and documentation guides, and BH screening, warm handoff/triage, and referral criteria documentation.
  9. Ensure continuous availability of the on-call “warm handoff” consultation service at IBH locations and communicate adjustments adequately and in a timely manner.
  10. Provide presentations and training to diverse stakeholders.
  11. Develop, implement, and evaluate interdisciplinary case conferences/rounds that meet the needs of individual primary care sites.
  12. Lead quality improvement evaluation and efforts using data and create ongoing quality improvement efforts to meet requirements for screening, intervention and services. Ability to analyze data and make data-driven changes and operationally efficiencies.
  13. Lead grant projects and contracts that apply to the IBH service. Facilitate or participate in associated meetings, quality improvement efforts with project managers and support staff. Ensure deliverables are met and required reports/data are submitted in a timely manner.
  14. Develop and maintain Continuous Quality Improvement (CQI) processes for clinical and case management functions and services; audit/review assessments, screenings, treatment plans, and progress notes for accuracy and adherence. 
  15. Track, monitor and report on data related to outcomes and outputs required by contracts.
  16. Attend county and community meetings as representative of CommuniCare Health Centers.
  17. Provide variety of direct services including: group and individual therapy, Warm Hand-offs, screenings and assessments, crisis management and triage, and coverage for services as needed.
  18. Maintain responsibility for remaining within the legal and ethical standards established in the field, including those of confidentiality. Maintain knowledge of best practices in integrated care, and lead efforts at the agency regarding integrated care and promising practices.
  19.  Ensure staff/trainee completion of IBH service onboarding and training; adherence to documentation, workflow, reporting, and productivity expectations; and comprehensive and continuous compliance with program requirements. This will be done via direct observation, individual and group supervision meetings, completion certificates, chart/administrative/data reviews, and timely feedback.
  20. Ensure clinical supervision and consultation support and training needs are met within critical timeframes for unlicensed IBH staff, and in accordance with applicable agency and professional regulations and/or staff development needs. This includes providing expedient clinical decision support and leadership for the IBH team in case management and crisis services/calls as needed.

  23. Manage IBH provider case assignments and maintain optimal provider productivity.

24. Actively contribute to quality assurance and improvement (QA/QI) projects and initiatives (i.e., Grant deliverables, projects, Peer Review/Chart Review).

25. Ensure maximal quality of services and compliance with the mission and strategic goals of the organization, federal and state laws and regulations, agency policy and procedure, accreditation standards, applicable professional standards, and supervisor/team expectation and/or assigned duties. This includes maintaining current professional knowledge and skill.

26. Work in strong collaboration with Chief Behavioral Health Officer, Chief Medical Officer, and other managers/supervisors.

27. Other duties as assigned by Supervisors.

Qualifications


Education:  LMFT or LCSW required

Experience:  A minimum of two years of experience working in mental health setting and two years of supervisor/lead/project leadership experience preferred.

Special Skills/Equipment: Bilingual Spanish/English preferred but not required. Must provide proof of current driver’s license and auto insurance and ability to travel in Yolo County. 

 

Expectations:  Attendance and punctuality is necessary.  Ability to work well with a multidisciplinary team. Understanding and use of DSM 5 and ICD10.  Computer proficiency at a level sufficient to complete the job duties.  Must have a commitment to excellence and high standards, excellent written and oral skills, strong organizational, problem-solving and analytical skills.  Must have ability to manage priorities and workflow.  Required to have versatility, flexibility and a willingness to work within constantly changing priorities with enthusiasm and have acute attention to detail.  Expected to have strong interpersonal skills and the ability to understand and follow written/verbal instructions. Able to work independently with little or no supervision. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers.

Ability to operate office equipment, including computer equipment.  Must be able to sit or stand for long periods of time, with occasional walking.  Ability to bend and lift up to 15 pounds.  Hearing and vision required. Work is performed in a typical clinic environment with moderate equipment noise. Occasional travel to other clinics and locations may be required.
