Job Title: Director of Population Health

Summary:
Under the direction of the Chief Medical Officer (CMO) the Director of Population Health supports the mission of Community Access Network to improve health outcomes by focusing attention on groups of patients as well as individual patients identified as having or likely to have gaps in care, adverse health outcomes, or suffer from health inequities related to social, racial/ethnic, environmental, and economic determinants of health. The Director of Population Health develops, implements and oversees programs within Community Access Network that facilitate reliable, patient-centered care that improves timely access to and provision of preventative services and chronic disease treatment. A vital member of the Leadership Team, this position provides strategic expertise and leadership to Community Access Network’s quality, patient engagement and patient-centered medical home initiatives. This role is critical in supporting internal and external health equity efforts to reduce disparities. The Director of Population Health leads the work of assigned staff to improve the health of individual patients and the community.
Responsibilities:
- Develops strategy for management of health center patient populations with chronic conditions and ensures delivery of evidence based care through a team-based, patient centered medical home (PCMH) model of care. Creates strategy goals to include improvement in quality measures, decrease in costs of care, and improved patient experience. Develops measurable objectives, sets goals, and reports relative to them.
- Responsible for a wide range of quality activities. Partners closely with Community Access Network leadership, providers and staff, playing a crucial role in performance improvement by identifying gaps and root cause operational issues, both through data and operational observation. Provides feedback and education, recommending actionable initiatives from the opportunities identified.
- Provides leadership to support financially sustainable population health initiatives. Ensures consistent operational processes, patient engagement and targeted prevention of chronic disease.
- Ensures adequate management of assigned staff, including establishing workloads, prioritizing work assignments, establishing performance expectations, and evaluating employee performance; enforcing policies and procedures; resolving staff issues; assessing training needs, monitoring program performance; and evaluating the effectiveness of services
- Oversees, directs and supports the Patient Engagement Coordinator and other assigned staff.
- Utilizes eClinicalWorks, i2i and other available population health registries to manage patient data, generate practice reports, and to identify opportunities to improve both clinic workflows and data quality accuracy.
- Utilizes dashboard for tracking effectiveness of programs and/or pilots. Performs detailed analyses and provides recommendations for continuous improvement activities, based on the data collected.
- Leads activities to achieve quality performance goals and to close care gaps by serving as a subject matter expert on quality measure specifications such as UDS measures; ensuring the optimized use of population health registries and eClinicalWorks to generate and deliver population health outreach reminders; and supports annual HEDIS Audit data reporting to payers.
- Responsible for leading the initiative to become a Patient Centered Medical Home, as well as Distinction in Integrated Behavioral Health. Learns and maintains expert level knowledge of the NCQA PCMH recognition requirements.
- Increases continuity of care by managing relationships with secondary and tertiary care providers and referrals.
- Collaborates with other Leadership Team members to develop pilot projects and scale up population health initiatives at Community Access Network.
- Works with team members to develop step-by-step procedures for complex processes. Trains staff in related policies and procedures.
- Collaborates with other departments to improve efficiency and care coordination. Communicates with staff and is responds quickly to internal and external customers. Shares information and own expertise with others to enable accomplishment of goals and objectives.

Qualifications:
- Bachelor’s degree required, master’s degree in a relevant field preferred.
- Minimum 5 years of relevant experience in healthcare, continuous improvement and/or Population Health.
- 3 years staff supervision experience required.
- Demonstrated experience leading multidisciplinary teams or projects.
- Demonstrated proficiency in data management, analytics, and reporting required.
- Ability to work harmoniously and effectively with colleagues, patients and other stakeholders across the spectrum of diversity.
- Experience with electronic health records required, eClinical Works preferred.
- Strong organizational, communication, presentation and leadership skills.
- Broad understanding of community health centers, health inequities and evidence-based population health strategies.

Salary: $70,000.00 - $90,000.00 per year, depending on qualifications
