Community Care is hiring a Case Manager/Care Manager to join our Family Care Program Team. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. It is based in Kenosha County, with telework currently being the norm.
Case Manager/Care Manager Job Responsibilities:
Manages an assigned caseload of program members in collaboration with an RN Care Manager
Completes comprehensive member assessments, member centered care plans, & functional screens
Provides ongoing care coordination and quality assurance monitoring of services
Maintains required documentation in adherence to established standards.
Case Manager/Care Manager Job Requirements:
Education: Bachelor’s Degree in a Human Services, Psychology, Social Work, or closely related field.
Degree Substitution: Applicants without a degree in human services, psychology, social work, or a closely related field, may still qualify if they have a minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities. For example, if someone has a degree in health care management or business administration, they may be qualified if they have at least 3 years of experience working with seniors or adults with physical and/or intellectual disabilities.
Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity required.
Certification: Eligibility for long-term care functional screener certification is required
Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Care Manager Job Schedule:
Monday to Friday
Generally during business hours of operation (8AM-4:30PM)
