We have a rewarding opportunity for an Occupational Therapist to work with our PACE program. This position is responsible for ensuring that basic principles of the managed care program are delivered to members via high quality, effective and cost effective methods. This is an opportunity to participate in a collaborative mix of interdisciplinary professionals while overseeing delivery of individualized therapeutic interventions. Delivery of care includes a mix of home health, sub-acute, and outpatient models.. No weekends, holidays or on-call.Occupational Therapist Job Responsibilities: ·
Complete all required documentation and charting and assists in the development and maintenance of member records·
Conduct instruction to staff, members and caregivers on Rehab services, programs, treatments, equipment and body mechanics as needed.·
Collaborate as a member of care team(s) and participates in team meetings, including care planning, service/resource allocation and updates on functional mobility.Job Requirements: ·
Experience: A minimum total of 1 year of personal, clinical/internship and/or employment experience providing care or services for adults with disabilities or frail elderly population is required. Preferred: Prefer 5 years of personal, clinical/internship and/or employment experience providing services for adults with disabilities and/or frail elderly population. Case Management experience. ·
Education: Bachelors in Occupational Therapy required. Masters in Occupational Therapy preferred. ·
License/Certifications: Occupational Therapist license, CPR & AED certification·
Travel: Requires use of a personal vehicle and valid driver’s license for occasional home visits and travel between Community Care sites. Job Schedule:
