POSITION DESCRIPTION

The Multipurpose Senior Services Program (MSSP) embraces a compassionate, nonjudgmental, and multidisciplinary team approach aimed at facilitating comprehensive care management. Our mission is to support low-income, frail elderly individuals in maintaining their independence and quality of life within their own homes.

As a vital member of our team, the Nurse Care Manager (NCM) plays a pivotal role in conducting thorough health assessments of program participants as well as assessing potential participants’ level of care. Moreover, the NCM is entrusted with the development and implementation of tailored health interventions to foster independent living.

Collaboration lies at the heart of our approach, and the NCM works closely with Social Work Care Managers and other team members to conduct comprehensive needs assessments. Together, they formulate holistic care plans, coordinate services, and diligently monitor and assess needs. Additionally, the NCM serves as a crucial liaison between MSSP and the medical community.

EDUCATION & EXPERIENCE

RN certificate and current California License in good standing. Additionally, one year of clinical nursing experience, preferably with frail older adults, or one year of case management experience. Additional geriatric/community health experience a plus.

REQUIRED KNOWLEDGE, SKILLS, & ABILITIES

Ability to organize time and set priorities to certify Level of Care determinations, collaborate on Care Plans, and implement services while maintaining deadlines and program requirements.

Skilled at using sound judgment in problem solving, and identifying and developing support systems for the participant.

Ability to communicate complex situations orally and in writing using computers, software, and related equipment with attention to detail.

Works well as a member of a team, and demonstrates skills at cultivating and maintaining cohesive working relationships with coworkers.

Sensitivity and awareness of participant’s rights, wishes and needs, while also showing the ability to maintain the boundaries of a professional relationships with participants.

Responsibilities include geriatric assessments of medication, physical, psychological, and medical needs. Ability to monitor participant’s needs and provision in the care plans.

ROLES & RESPONSIBILITIES

The NCM must evaluate the potential participant as a total person and identify the functional and health limitations that impede independent living.

The NCM is responsible for the following activities:

  • Conduct in-depth assessments and/or reassessments covering medical, health, and rehabilitation concerns.
  • Certify level of care determinations.
  • Consult with MSSP team to create Care Plan collaboratively.
  • Complete reports and assessments within deadlines required.
  • Implement services detailed in care plan.
  • Monitor participant’s needs and provision in care plan.
  • Collaborate with physicians and other health professionals.
  • Ensures that prior to purchasing and authorizing Waiver Services, all other resources have been exhausted.

From time to time, employees are asked to perform duties and handle responsibilities that are not in their job descriptions. If, over the months, the new duties and responsibilities remain a significant part of the assignment, the job description is changed.

Job Type: Full-time

Pay: $35.15 - $42.01 per hour
