Job Posting for Bilingual Patient Advocate at Community Clinic
Community Clinic is seeking full-time Patient Advocates to work in our medical clinics. The Patient Advocate is a member of the patient care team and possesses a strong understanding of the community served. This position serves as a link between the patient and health/social services in the community, provides resources and connections that focus on lifestyle modification, communicates with medical providers, and educates patients regarding the importance of wellness and chronic disease management.
Bilingual English/Spanish is required. Must be proficient in Microsoft Word and Excel and have reliable transportation. A bachelor’s degree in a Human Service field is preferred, but applicable experience in a community service role will be considered. This is a full time position, M-F, 8-5.
