What are the responsibilities and job description for the SUD Case Manager position at Community Health Alliance?
The Case Manager serves as a community liaison aiding clients with accessing community resources for individuals experiencing substance use disorder through our Sojourner Recovery Services agency.
Job Duties:
- Assist clients in obtaining services, housing, and employment through community resources.
- Provides support, guidance, and transportation for clients to all necessary appointments.
- Provide and/or arrange for crisis intervention for clients in a crisis.
- Position requires frequent driving and walking.
- HS diploma or associates degree in social sciences field or its equivalent.
- Customer service skills.
- Communication skills.
- Valid driver's license, insurance, and insurable driving record required.
