JOB TITLE: Outreach Worker
JOB OVERVIEW: The Outreach Worker is responsible for patient advocacy and assistance, including advising patients regarding available community resources and facilitating access to these services. Assists patients in State medical/dental coverage enrollment process, referrals, and completion of forms. Promotes utilization of primary health care. Acts as clinic resource regarding State medical/dental coverage.
ESSENTIAL FUNCTIONS INCLUDE, BUT ARE NOT LIMITED TO:
- Patient Advocacy/Assistance
- Facilities client access and follow-up to clinic and other necessary services.
- Assists patients’ enrollment in State medical/dental coverage by complete and accurate applications to the most appropriate plan
- Process Patient Referrals to Specialists
- Directs patients to appropriate community services and resources as needed
- Arranges for transportation for patients as needed
- Compiles and distributes pamphlets and literature for patient/community education on State medical/dental coverage, health care, and community resources. Presents such information in oral presentation for organizations and at public events as assigned
- Arranges for translation for non-English speaking patients or provides translation as requested and able
- Provides patients with information and assistance in completing necessary forms as requested
- Client Tracking and Follow-up
- Facilitates client access to the clinic and promotes utilization of preventative health service. Monitors and tracks utilization as requested
- Promotes primary care follow up care through patient contact after their emergency room visit
- Serves as clinic resource on State medical/dental coverage programs and requirements
- Assists in preparation of “no-show” and “inappropriate behavior” patient letters as requested
QUALIFICATIONS:
Education:
High School diplomas or equivalent
Experience:
At least 1-year previous experience in related field
