We are seeking a full-time Cancer Screening Navigator. This role is that of consultant, educator and manager of an ongoing Colorectal Cancer Screening Program with a multidisciplinary CHCNRV care team. This position will help lead internal change process as well as be involved with the implementation of other evidence-based Cancer Screening programs. For the current CRC screening program, the Navigator will provide support and develop staff and provider education training related to all processes, screening guidelines, and CRC workflows across clinic sites. The Navigator will also work closely with the Care Coordination team to provide patient support, education, coordination of care, and other assistance as needed.
For future Cancer Screening programs within the CHCNRV, the Navigator will assist with conducting an internal system-wide needs assessment to verify that the cancer screening strategies meet the needs of the CHCNRV. Using assessment outcomes, the Navigator will develop a sustainability action planning process to promote long-term impact of the Program.
With respect to patient care, the Navigator will act as a liaison between the CHCNRV and other ancillary services to ensure the needs of patients are being addressed. The Navigator orchestrates timely, effective referral and follow-up communications between referring providers, multidisciplinary specialists, and the patient’s support system as well as the community during cancer screening, community forums and support group activities. This is an integral part of the role.
Qualifications – Education: Registered Nurse, BSN preferred. Experience: Minimum of 3 years of experience in a community-based outpatient setting or hospital. Experience as a navigator and patient educator in oncology preferred. Experience in obtaining insurance authorization for services preferred. Ability to organize group meetings and public speaking a must. Demonstrated sensitivity and commitment to mission, core values and program. BLS certification required.
