APP Salary Range:$99,000 To 130,515.84 Annually (Based on 0.7 FTE)
MD/DO Salary Range: 166,000 to 196,560 Annually (Based on 0.7 FTE)

JOB SUMMARY:
OLE Health is looking for a Mobile Health Unit Provider who is bilingual in English/Spanish. Family Nurse Practitioner (FNP) or Certified Physician Assistant (PA-C) is preferred. The Mobile Health Unit (MHU) Provider will be responsible for providing quality primary health care to patients in a mobile health setting as well as on site coverage if absence of mobile health. The MHU Provider will travel extensively with the mobile health team in both Napa and Solano counties providing care to the community at large, including, but not limited to, farmworkers, unhoused, student, seniors, and other vulnerable populations. The Mobile Health Unit Provider will be working closely with other healthcare staff in a team approach.

DUTIES AND RESPONSIBILITIES
Travels extensively to community locations and other outreach destinations with OLE’s mobile health team at least two to three days per week – evenings, and weekends as needed. Potential to provide care within health center should Mobile Health not be active (example, but not limited to weather, vehicle maintenance, unforeseen reasons) |
Provide the following health care services: assessment/evaluation, diagnosis, treatment/prescribing, planning and education. Consult with staff physicians, pharmacists or other care team members as needed. |
Review and manage patient information. |
Prescribe and administer medications and treatment regimens according to assessed medical conditions. Review medication(s) that patient is currently taking, prior to prescribing to prevent adverse reactions. |
Review and manage patient information including labs, diagnostic images, prescription refills, outside consultation notes, previous medical records, specialist referrals, etc. |
Refer patients to specialists and to relevant patient care services as appropriate. |
Perform various outpatient procedures as indicated, appropriate, or per privileging. |
Prioritize patient care throughout the day. |
Provide acute triage as needed. |
Provide education and information to patients on various diseases, providing handouts as appropriate. |
Perform various administrative procedures as directed and complete various medical related forms as needed. |
Document all appropriate information in electronic health record system. |
Respond to messages and relay information to patients and other external sources as needed. |
Direct and coordinate the patient care activities of support staff as appropriate. |
May participate in and collaborate with staff/teams on various support-related functions including quality assurance, care team development, system optimization, empanelment, policy and procedure development, etc. |
Assists in the coordination and integration of all health clinic outreach programs as directed |
May be required to respond to afterhours outpatient triage calls on a rotation basis. |

EDUCATION, EXPERIENCE, TRAINING
- NP or PA-C with at least 3 years of experience preferred.
- MD or DO with at least 1 year of experience.
- Graduate of an accredited Master of Science in Nursing program required, or graduate of a Physician Assistant Program from an accredited school, or MD/DO licensed in California with board certification.
- Current Family Nurse Practitioner certification required (for FNP position)
- Current Physician Assistant License required and National Commission on the Certification of Physician Assistants Certificate required for PA applicants
- Current California Provider certification/licensing required.
- Must have or obtain current California medication furnishing license.
- Must have or obtain current DEA License.
- Buprenorphine Waivered Practitioner, or obtain within 4 weeks of hire
- Must certify and remain current in CPR certification.
- Experience in a community healthcare center, or mobile health unit preferred.
- Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred.
- Excellent interpersonal and customer service skills required; must have ability to treat patients with complex medical and social needs.
- Must have strong time management skills, ability to prioritize, and work in a fast-paced environment.
- Computer and data entry skills required; electronic health record system preferred.
- Excellent problem-solving skills required.
