What are the responsibilities and job description for the HIV OUTREACH WORKER I position at Community Health of South Flor?
POSITION PURPOSE:
To find clients new and lost to HIV care, help to overcome barriers and gain access to HIV treatment. To outreach into the hard to reach communities and identify potential HIV clients.
Determines client's needs and Coordinates primary care appointments and follow-ups. Completes Outreach intakes package and documentation which enables client to access other Ryan White services.
Develops a working relationship with the various clinics and hospitals, community health centers, and, community social services centers as a source of referrals for patients.
Assist clients with information about PrEP and how to receive services, coordinating with the HIV Navigator/PrEP Educator.
POSITION REQUIREMENTS / QUALIFICATIONS:
Education/Experience:
High School graduate or GED equivalent. Prefer an AA degree. Minimum of six months outreach experience. Familiar with the Homestead, Naranja, Goulds, Richmond Heights and Perrine geographic area and those in the community.
Licensure / Certification:
CPR Certification from the American Heart Association. Must have an active Florida Driver’s License. HIV/AIDS 501 & Ora Sure Certification desired.
Skills / Ability:
Must have reliable transportation, good communication, writing, and inter-personal skills. Must be multi-lingual and multi-culture.
POSITION RESPONSIBILITIES (THIS IS A NON-EXEMPT POSITION)
To find clients new and lost to HIV care, help to overcome barriers and gain access to HIV treatment.
To outreach into the hard to reach communities and identify potential HIV clients and those at risk of becoming HIV . Promote HIV testing, access and education about PrEP, and linkage to care.
Determines client's needs.
Coordinates primary care appointments and follow-ups.
Coordinates Primary Care working group consisting of both state and local HIV/AIDS service organizations whose function is to help steer the direction of the primary care liaison.
Develops a working relationship with the various clinics at hospitals, community health centers, community Social centers and case managers as a source of referrals for patients who may be lost to care at these locations.
HIV Outreach Worker I must monitor client’s adherence to treatment and Medication regimens.
Refer individuals to HIV providers and follow up on outcome of referrals.
Encourage individuals to become involved in consortium activities.
Participate in any other functions to meet the goals of this activity.
HIV Outreach Worker I must collaborate with other caregivers (i.e. medical care provider, Nutritionist, prescription drugs provider, etc.) to ensure that the client receives coordinated, interdisciplinary support for adherence and assistance in meeting treatment objectives.
Submit monthly/quarterly reports in a timely manner per contract requirements.
Meeting 100% of contract requirement.
Making home visit to patients who have not had any contact in 60 days as request by the Case Manager.
Collaborates with other agencies in Community Health Fair and events.
Develops relationships with drug treatment programs, testing sites, shelters, prisons, the homeless, etc.
Identifies additional ways and/or locations to make contacts with persons infected and affected by HIV/AIDS.
Performs other duties as assigned.
