Community Healthlink (CHL) recognizes the power of a diverse community and seeks applications from individuals with varied experiences, perspectives, and backgrounds.  Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Program Coordinator in Worcester.

 

Worcester Respite is a 3-bed DMH referred program. Respite clients reside on the unit for a set amount of time agreed upon by the treatment team and work towards their individualized treatment goals. Goals often include stabilization, housing, employment, and continued sobriety. Treatment team meetings are held with community providers, family and support systems, and Respite supervisors to assist individuals in meeting their treatment goals. Clients on the Respite program are encouraged to attend day programs, appointments in the community and to maintain daily structure.

Responsibilities

Essential Responsibilities:

  • Provide oversight of residential component.
  • Act as Program Director during his/her absence. 
  • Ensure operational budget for the program is reconciled. 
  • Orient and train new staff, as designated, to daily operations of the program and Division procedures.
  • Ensure new staff are oriented and trained as required by CHL.
  • Oversight of site based and mobile clients.
  • Facilitate biweekly meetings with external stakeholders.
  • Provide ongoing supervision to program staff. 
  • Facilitate the transition for referrals, admissions, and discharges.
  • Strive to maintain maximum capacity. 
  • Plan, implement and continuously evaluate rehabilitative and support services through assessments, treatment planning to meet individualized needs, and development of a peer support group. 
  • Take personnel action in regard to hiring and disciplinary procedures. 
  • Participate in on-call through the emergency beeper system. 
  • Serve as liaison among other departments and with other agencies in order to facilitate a continuum of care. 
  • Identify problem areas and coordinate work for upkeep of site.
  • Insure day-to-day upkeep of residence and workplace.
  • Administrative on call rotation.
  • Ensure accuracy and timeliness of resident calendar, admission, and discharge forms.
  • Special billing, and/or Service Activity Logs. 
  • Transporting residents in personal car and/or program van
  • Perform Other related job duties.

Standard Staffing Level Responsibilities:

  • Complies with established division and program policies, procedures, and objectives
  • Attends variety of meetings, conferences and trainings as required
  • Demonstrates use of quality improvement in daily operation
  • Complies with health and regulations and requirements
  • Maintains confidentiality
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

Qualifications

  • Bachelor’s degree in a related field. Will consider several years of experience in lieu of a college degree.
  • Current and valid United States driver’s license and ability to provide registered and insured automobile for work related purposes including transporting Persons in a personal vehicle
  • MAP, CPR, First Aid certifications must be completed within the first six months after hire date
