Community Horizons is a Support Broker Agency that focuses and specializes in assisting individuals through the recruitment process of hiring Self-Directed workers. We are Wisconsin based and serve multiple counties throughout the state. In addition to navigating the recruitment process, our Support Brokers can assist with guidance within self-directing care and employee relations.
A woman in the Fond du Lac area is actively seeking a new caregiver to help her Thursday, Friday, and Sunday mornings. Scheduling is flexible!
