The Pay Schedule – Up to $25 per hour.
Major Duties & Responsibilities
· Showering /Personal Care-Wednesday and Saturday
· Bowel Program
· Catheter Care
· Meal Prep
· Dressing Assistance
· Medication and Nebulizer set up—includes applying creams
· Light House Cleaning
· Laundry
· Local Errands
Schedule
Friday 8:30 a.m. to 12:30 p.m. and 4:30 p.m. to 7:30 p.m.
Every other weekend the same shifts.
Wage – Up to $25 per hour
The ideal candidate for this opportunity will have some experience, reliable, dependable, and a Yahtzee player.
Must be pet-friendly!

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or Iris(Include, Respect, I Self-Direct) throughout Wisconsin. Self-directing gives program participants the choice over who provides services and how they receive them.
