Fleming Island, FL 32003 The Community Relations Representative is responsible for representing CHPC to our community and educates them on advanced illness and our programs. The goal is to increase the awareness and knowledge about the services that CHPC provides patients, caregivers, family members and the community within a defined territory and within these territories, additional focus will be given to strategically identify diverse populations as determined by the organization. The Community Relations Representative is expected to be an active spokesperson at community events and meetings where appropriate and utilize the organization’s Ambassador Program to extend that reach. PRIMARY RESPONSIBILITIES: Develops community relationships with which you will engage…i.e. churches, civic organizations, senior centers, YMCA’s, etc. Develops community engagement plan in conjunction with the Community Relations Manager and team with emphasis on reaching family caregivers, seniors, veterans and other identified audiences. Achieves quarterly and annual outreach goals within assigned territories. Coordinates community activities with corresponding Healthcare Relations Representatives, and in collaboration with Marketing and Communications. Identifies opportunities for themselves, Community Hospice Ambassadors and other CHPC staff, as appropriate, to perform presentations on hospice care, advance directives, caregiving and other topics as requested. Facilitates speaking engagements involving other CHPC staff and others as appropriate. Coordinates participation in community health and vendor fairs and other appropriate events. Assessed and prioritizes event opportunities to make sure that coverage is provided for most important events. Supports the efforts of CHPC Foundation by managing community relationships and making appropriate introductions to prospective donors and volunteers. Maintains accurate and up-to-date records within assigned database. Completes appropriate documentation, to include tracking and outcomes of all activities. When appropriate, assist community members by facilitating process to arrange for informational visit or follow-up to referral requests. Promotes CHPC through participation in community and professional organizations such as the Chamber of Commerce, etc. Complies with CHPC policies, procedures and standards of practice. Improves own professional knowledge and skill level on issues important to CHPC. Other additional responsibilities, as determined. REQUIRED LICENSES/CERTIFICATIONS: Current State of Florida driver’s license with driving record acceptable to CHPC insurance carrier, and reliable transportation. QUALIFICATIONS: EDUCATION/EXPERIENCE: Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to: Bachelor’s degree in marketing, communications or a related field. At least three (3) years’ experience in marketing, sales and /or community outreach. Clinical knowledge and/or background a plus. Fluent English, Spanish or other secondary language is a plus. Knowledge of and existing community relationships with geographic area of responsibility a plus. SKILLED AT: Strong interpersonal and communication skills, both verbal and written. Ability to build relationships and influence others in a public speaking environment. Ability to communicate and articulate CHPC services and benefits to members of the community in a clear and concise manner. Excellent presentation skills for both large and small audiences. Demonstrated ability to work effectively with a wide range of constituencies in a diverse community. Ability to work autonomously, support the needs of CHPC, fellow team members and Healthcare Relations Representatives. Computer proficiency with Microsoft Office, customer relationship management software a plus.
