Position: Nurse Practitioner
Responsible to: Chief Medical Officer/Medical Director
FLSA Status: Exempt
**Coverage Areas: This position will cover multiple counties, which may include Columbiana, Stark, Carroll, and Tuscarawas. We offer mileage reimbursement.
POSITION SUMMARY
Promotes advanced nursing practices in the hospice and palliative care setting by: 1) directing and evaluating patient care; 2) providing consultations to the nursing staff; 3) collaborating with the medical staff of hospice and in the community; 4) maintaining professional knowledge and skills; and 5) acting as a role model for other nurses.
PRIMARY RESPONSIBILITIES
- Initiate palliative care consultations or appropriate hospice treatment plans for patients in all settings. This may include history, physical examination; interpretation of health data; prescribing medications in accordance with state law; ordering routine and diagnostic studies; and collaborating with the physician as appropriate.
- Assist with determination of hospice eligibility.
- Participate in Interdisciplinary Team as needed.
- Assist with clinical orientation and volunteer training as needed.
- Assist in the development of clinical policies and procedures as needed.
- Complete face-to-face encounters and corresponding documentation.
- Coordinate with other clinicians in providing patient care.
- Involve patients/families in care planning process; responds to needs.
- Provides nutritional education and recommendations as appropriate.
- Document time and billable hours appropriately.
- Participate in medical call rotation, if assigned.
- Participate in performance improvement activities.
- Participate in community and agency functions that foster the mission of hospice and Palliative Care (public speaking, professional organizations, fundraising, etc.) as needed.
- Contribute to a culture of mutual respect, integrity and serving each other.
- Acts in compliance with federal and state regulations, and with the policies and procedures of Community Hospice.

JOB SPECIFICATIONS
- Graduate of an accredited school of Nursing. Graduate of an approved Nurse Practitioner Program (MSN)
- Licensure: RN license to practice nursing in the State of Ohio. Certified Nurse Practitioner designation.
Experience: Minimum of 3-4 years experience as a Registered Nurse; 2-3 as a Nurse Practitioner preferred.
Skills: Ability to identify and resolve problems in a timely manner; to gather and analyze information objectively; to develop alternate solutions as needed; to assess and respond quickly in emergency situations. Exhibit sound judgment and require minimal supervision.
Interpersonal Skills: Good interpersonal skills in dealing with patients, families, physicians, public, volunteers and the Hospice Staff. Demonstrate flexibility, responsibility, dependability and the capacity to work in an interdisciplinary setting.
Physical: Able to drive an automobile and travel up to see families in all locations served by Hospice. Able to stand for moderate periods of time; able to sit for extended periods of time; able to bend, push, lift, reach, grasp, and release objects; able to assist with the moving of objects over 25 pounds; able to climb steps, hills and uneven surfaces.
Mental: Able to assess, interpret, comprehend and evaluate varied data; teach/explain data, concepts and ideas to groups of people; retain information on a short- and long-term basis; comprehend spoken, written and nonverbal communications; respond in a positive manner to changing circumstances; delegate duties when indicated; make decisions and problem-solve; participate in meetings; communicate ideas clearly both verbally and in writing. Able to deal with the stress of dealing with terminally ill patients and their families as they prepare for end-of-life issues.
Exposures to Hazards: Infectious diseases, driving hazards, animal bites and inclement weather.
Other: Dependable automobile and current Ohio driver's license without restrictions
Proof of active automobile insurance.
Satisfactory criminal background report.
