Summary:
Provides assessment and clinical treatment to all clients presented to the clinic by utilizing various
treatments inclusive of individual, family and group therapy to address identified client needs.

Essential Responsibilities and Target Outcomes:
Completes assessment tool, integrates pertinent information to establish diagnosis, and

analyzes data to create clinical formulation within designated timeframe.
 Develops achievable goals with client; with measurable and behavioral objectives within

designated timeframes. Monitors treatment progress and provides updates within identified
timeframes as necessary.

 Performs individual, family and group treatment utilizing evidence based practices to facilitate
positive strength-based change.

 Collaborates and coordinates with external agencies such as schools, pediatricians, DCF, juvenile
court and other social service agencies in a timely manner.

 Responds to crisis situations and triages appropriately, communication clearly with professionals
– internal and external – to meet presenting client needs.

 Provides referrals and linkage to community and internal resources as necessary to address
client need.

 Secures reimbursement by obtaining authorizations, providing accurate documentation and
submitting progress notes in a timely manner. 

Completes documentation with designated timeframes as established by CMHA and external
regulatory agencies.

 Performs other related duties as assigned.
Principal Customers:

Children, Adolescents, Families and Adults referred by DCF, Parents, Schools, Court System,
Pediatricians, other programs within CMHA and other social service agencies seeking mental health
and/or co-occurring treatment.

Critical Work Relationships:
Directors, coordinators, program clinicians, case managers and psychiatrists, other programs within
CMHA , other external providers and social service agencies as appropriate.

Minimum Requirements:
Education: Master’s degree in behavioral health (psychology, social work, counseling,

marriage and family therapy, and/or alcohol and drug counseling)

Credentials and
Licenses:

 License eligible (LPC, LMFT, LCSW) required.
 Licensed (LPC, LMFT, LCSW) preferred.
 Certified in first aid and CPR
 Bi-lingual (Spanish/English) preferred.

Experience:  Demonstrates desire to work with children, adolescents, families and
adults with moderate to severe behavioral health issues.

 Successful experience in the provision of mental health services with
children, adolescents, adults and/or families; preferably a minimum of
two years.

 Demonstrates beginner to intermediate experience with Microsoft
Office products including Word and Outlook. Experience in, or the
ability or willingness to learn how to use an Electronic Medical Record
(EMR) system which includes appointment scheduling, authorization
tracking, assessments, treatment plans, progress notes and discharge
summaries.
