SUMMARY OF DUTIES AND RESPONSIBILITIES

Community Overcoming Relationship Abuse (CORA), is the only agency in San Mateo County providing crisis intervention and supportive services to survivors of domestic violence and their children. The organization seeks a Bilingual Clinical Case Manager to provide trauma-informed clinical case management for participants in the Families with First Five Family-Centered Mental Health Program.


POSITION SUMMARY

This full-time position will provide trauma-informed clinical case management for participants in the Families with First Five Family Centered Mental Health Program.

This Clinical Case Manager will conduct therapeutic contact with clients and support intakes, screen participants, develop case management plans, provide referrals. They will also schedule and coordinate the logistics of the Family Engagement Groups and Activities. This position will be supervised by the Coordinator of the First 5 program and work closely with the clinical therapy team.

 

Primary Job Responsibilities

  • Conduct case management intakes, screening, assessments and crisis case management for family survivors of Domestic Violence; Parents and Children 0-5
  • Provide administrative support, coordination and logistical preparation for all Family Engagement Activities and Groups
  • Provide immediate trauma-informed domestic violence therapeutic support as needed and appropriate.
  • Receive, coordinate, provide referrals, for the program to ensure the integration of a coordinated system-wide response to Families with children 0-5.
  • Meet with FCMH clients to create a case management plan.
  • Provide clinical support to assist clients in meeting their treatment goals.
  • Proactively link clients to both internal and external resources based on their treatment needs, walk families through the referral process as needed (including accompaniment to services), and follow-up on the progress/status of the complete referral process.
  • Facilitate Empowerment Groups and Activities for community members.
  • Provide follow-up consultation, as needed.
  • Document recommended case plan and activities in client record and other data management systems on a timely manner.
  • Serve as a liaison with the clinical therapy team, participants and clients, and a wide variety of community service providers.
  • Participate in meetings and trainings as requested.
  • Maintain confidentiality and exercise professional judgment at all times in keeping with the responsibility carried personally and by the agency for the care and welfare of youth and families.
  • Maintain and exercise cultural awareness and sensitivity to diversity from a trauma-informed lens.
  • Other duties as assigned.

 


Requirements

  • Either a Bachelor’s degree from an accredited institution in social work psychology, counseling and a closely related field or 3-5 years of clinical case management experience in a mental health setting. The experience must have included screening, assessment, treatment planning, providing referrals, and follow up.
  • The experience should include at least 2-3 years of experience providing case management services to children, families, and at-risk populations.
  • Bilingual in English Spanish, both verbal and written, preferred.
  • Ability to conduct intake, develop case management plans, and provide follow-up and referrals for families and caregivers.
  • Able to effectively handle mental health crisis, establishing priorities for timely service delivery, and work as a team with the clinical therapy staff in the midst of crisis.
  • Successful completion of a certified domestic violence counselor training course (may be completed upon hire).
  • Demonstrate cultural competency skills and sensitivity to diverse cultural values and norms.
  • Ability to write concise reports and evaluations.
  • Excellent judgment and decision-making skills, adherence to program protocols, and maintenance of professional boundaries.
  • Ability to work with community collaborators. Knowledge of San Mateo County community resources preferred.
  • Intermediate - advanced computer skills, including use of Microsoft Office 2010 suite and other computer programs related to service delivery, with ability to learn new software, as needed.
  • Valid driver’s license, auto insurance, and vehicle are preferred.
