We are looking for a dedicated Case Manager/Rehabilitation Specialist for our Brain Injury Support Services program in Greenville, NC to provide a holistic approach to employment and increased independence through the Brain Injury Support Services program (BISS). The BISS program helps people who have sustained brain injuries increase their independence and find jobs that match their career goals.

KEY JOB RESPONSIBILITIES INCLUDE:

1. Conducting intake admissions process for new referrals as required by appropriate funding sources and by Community Partnerships’ established policies and procedures.

2. Performing initial psychosocial assessments for each new consumer to identify emotional, social, and environmental strengths and weaknesses related to their brain injury and other diagnoses.

3. Collaborating with individual consumers, agency staff, and other community resources to develop, implement, and evaluate Person Centered Plans to target cognitive rehabilitation, therapy and counseling, case management, and job acquisition and retention skills.

4. Providing individual therapy to consumers regarding cognitive, behavioral, emotional, and/or vocational concerns.

5. Providing individual instruction to consumers to strengthen, modify, and/or compensate for cognitive deficits.

6. Developing cognitive retraining strategies utilizing computer and adaptive equipment to enhance consumer's strengths and skills.

7. Providing informal training to staff, families, and related professionals.

8. Instructing consumers in basic job seeking skills, such as interviewing, resume development, and networking as appropriate.

9. Analyzing potential employment situations for job skill requirements and performance expectations, need for employer/co-worker education, need for assistive technology or job task modification, and coaching/support needs for on-the-job training.

10. Completing job development activities including completing labor market surveys, reviewing job postings, and contacting employers to explore opportunities in the community

11. Providing vocational training on job/task performance (using coaching and one-on-one instruction, etc.) to ensure the consumer is able to reach and maintain the employer’s required level of proficiency.

12. Providing follow-up support and monitoring work performance/progress through regular contacts with the employer and consumer during the initial 90 days of a consumer’s employment as well as the duration of the employment.

13. Providing case management services for continuity of care

MINIMUM REQUIREMENTS:

Education: Bachelor’s degree in rehabilitation, psychology, special education, social work, or related field required. Graduate degree in rehabilitation, psychology, special education, social work, or related field strongly preferred.

Experience: Two (2) years of case management experience and two (2) years of direct experience working with people with neurological disorders preferred. Relevant experience with other populations considered.

OR any equivalent combination of the above listed requirements.

Valid driver’s license and driving are required to meet the requirements of the position.

OTHER QUALIFICATIONS:
Ability to relate well to a variety of individuals and community groups and to work effectively with a multidisciplinary team. Ability to define work priorities, manage time efficiently, and work independently with minimal supervision. Comprehensive knowledge of community resources and the ability to access such. Knowledge of neurological disorders and cognitive re-training strategies. Knowledge of and ability to implement basic counseling and crisis intervention techniques. Knowledge of family dynamics and systems theory.
