What are the responsibilities and job description for the LPN - Community Behavioral Health position at Community Support Services Inc?
The Opportunity: We are seeking to hire an LPN Community to join one of our regional treatment teams. This individual is an integral part of the treatment team, works closely with the behavioral health provider, primary care providers, behavioral health team members, and other community agencies. The LPN assists the RN Care Coordinator with coordination of multiple health care services and consults, medication management, ongoing evaluation of client progress, re-enforce education, and provide feedback to the entire treatment team.
What Youll Do:
- Assist the RNCC with identifying those individuals with identifiable risk factors (at risk in levels or blood pressure and weight/BMI risk level).
- Act as liaison between public and private health facilities and service agencies to carry out referral instructions.
- Participate in an interdisciplinary treatment team, cooperate with and utilize the resources of other community agencies as described above, including other CSS programs, in order to achieve treatment goals of individuals served.
- Serve as liaison consultant to behavioral health treatment team on medication and healthcare issues.
- Make home visits to administer medication and/or assess individuals condition of health or provide other interactive services as directed by the RNCC.
- Accompany clients to medically complex medical appointments (specialists or primary care appointments.)
- Provide supportive interaction and education as needed to foster self-management skills and accountability.
- Maintain current and accurate client clinical records.
- Assist the Director of Nursing in maintaining clinical staffing within the medication clinic if needed.
- This position is full-time (40 hours a week) working normal business hours.

What Were Looking For:
- Graduation from an approved school of Practical Nursing leading to the attainment of Practical Nurse license required.
- A minimum of one years experience working with individuals who have a severe or persistent mental illness preferred.
- Current knowledge of psychotropic medications, including side effects, required.
- Recent clinical experience required.
- A car and a valid Ohio drivers license and insurability through the Agencys insurance carrier required.
- A deeply held belief in CSSs recovery-centered mission.
- Ability to develop positive relationships with individuals with mental illness, developmental disabilities, and substance abuse challenges.
- Skill in assessing individuals for strengths, needs, abilities, and preferences.
- Excellent organizational, attention to detail and communication skills.
- Ability to work in a team environment.
- Proficiency in Microsoft Outlook.
- Skill in managing time and meeting deadlines.
- Ability to negotiate assertively from an advocacy perspective.
- Knowledge of community resources and demonstrated ability to access.
