Position Concept: The Day Habilitation Coordinator (DHC) is tasked with marketing, recruiting, and retaining day services attendees. This role ensures compliance with regulatory requirements associated with service delivery, focusing on quality, safety, compliance, and retention.
Position Responsibilities:
- Marketing and Recruitment:
- Directly market and communicate recruitment opportunities to Care Coordination Organizations (CCOs), family advocacy, and other natural resources.
- Schedule, conduct, and follow up on program tours.
- Establish communication between the day services program, program service participants, families/advocates, and care management.
- Plan and attend marketing or recruitment events for Day Habilitation Services.
- Develop and oversee recruitment tactics for Day Habilitation services.
- Intake Process:
- Oversee and implement the intake process for program service participants in the Day Services Division.
- Serve as a direct liaison with families, advocates, and Care Management throughout the intake process.
- Ensure all person-specific trainings are addressed before the 30-day review.
- Supervision and Planning:
- Supervise program service participants' initial staff action plans, dining plans, and other person-centered plans through the 30-day, 6-month, and annual review process.
- Complete and track DDP1s for both the intake and discharge process.
- Communication and Assessment:
- Communicate with care management, parents, and/or correspondents of program service participants for planning service objectives, growth of services, and assessing progress.
- Facilitate quarterly fire drills for site-based services, working alongside Day Service Managers to address any identified areas of vulnerability.
- Fiscal Billing and Reporting:
- Assist with fiscal billing and training reports to ensure timely reporting and documentation for OPWDD and Medicaid requirements.
- Records Compliance:
- Quarterly audit site-based records to ensure compliance with OPWDD regulations and Medicaid billing standards.
- Partner with Day Services Manager (DSM) and Associate Day Services Manager (ADSM) to address areas of vulnerability.
- Quality Assurance:
- Participate in a systematic approach to complete plans of corrective action for discrepancies found in service satisfaction surveys, fiscal audits, and other audits.
- Policy and Procedure Updates:
- Review and recommend updates to any policy and procedure related to Day Habilitation in coordination with OPWDD regulations and Medicaid billing standards.
- Strategic Planning:
- Participate in strategic planning to assist progress, service appropriateness, and/or develop program strategies.
- Training and Meetings:
- Take part in any in-service training offered by the agency.
- Attend meetings as requested.
- Other Duties:
- Perform other duties and responsibilities as requested by the Manager and Management Team.
- Cooperation:
- Actively cooperate with colleagues and stakeholders.
