Community Youth Services’ Integrated Counseling Division is looking for compassionate and dedicated individuals to join our team as Care Coordinator with the Transitional Age Youth team.  The TAY Care Coordinator is an integral member of a specialized treatment team, delivering comprehensive care coordination services to youth aged 15-21 in Thurston and Mason counties. This role is crucial in equipping young individuals and their families with the necessary tools and strategies to effectively navigate and manage their involvement in various systems, ultimately leading to self-directed care. With a commitment to individualized service and a flexible schedule that accommodates nights and weekends, the Care Coordinator ensures accessibility and meets youth in their own environments. Trained in a suite of evidence-based practices and assessment tools, the Care Coordinator is adept at fostering a supportive network, advocating for healthy relationships, and catalyzing positive change. Qualifications:

  • Bachelor’s in Social Work, Psychology, Counseling, or a related field, or an Associate’s with two years of experience in care coordination or case management. Must have Agency Affiliated Counselor credentials in Washington State.
  • Familiarity with Mental Health, Juvenile Justice, Schools, DCYF, DSHS, DVR, and community support systems.
  • Understanding of cultural issues affecting marginalized youth, commitment to Positive Youth Development and Wraparound principles, and a non-judgmental approach.
  • Knowledge of legal processes concerning children and families, including dependency orders and juvenile court procedures.
  • High level of initiative, efficiency, organizational skills, and the ability to meet deadlines.
  • Proficient in leading meetings, public speaking, task coordination, and building diverse relationships.
  • Skilled in Microsoft Office, particularly MS Word and Outlook, and adept at using the internet and intranet.

Key Responsibilities:

 

  • Provide comprehensive Care Coordination in home, school, and community settings, adhering to Wraparound principles.
  • Manage a caseload of 10-15 youth, focusing on their needs and strengths.
  • Organize and lead Wraparound Team Meetings, ensuring inclusive and outcome-driven agendas.
  • Foster a supportive team culture, offering a safe space for youth and families.
  • Maintain a positive and accepting approach towards youth and family choices.
  • Exhibit leadership and conflict resolution skills to integrate diverse viewpoints.
  • Keep accurate records as required by “Managing and Adapting Practices” and associated networks.
  • Participate in necessary meetings and consultations, offering flexible availability.
  • Enhance natural support networks for youth and families within the community.
  • Ensure consistent communication and collaboration with Wraparound teams and partners.
  • Provide tailored resources and information to families for a clear understanding of the WISe process.
  • Apply consensus-building and conflict management to develop unified care plans.
  • Document all processes and progress in line with prescribed guidelines.

Agency Qualifications:

  1. Successfully clear a thorough background check with no history of causing harm, particularly to children or vulnerable adults.
  2. Maintain a clean criminal record, demonstrating professional and personal integrity.
  3. Hold a valid Washington State Driver's License with at least five years of driving experience and a good driving record for the past three years.
  4. Provide proof of auto insurance with coverage of $100,000 or more.
  5. Show cultural sensitivity and the ability to respond to the diverse needs of the organization’s community.
  6. Acquire CPR/1st Aid/Bloodborne Pathogen certification within 30 days of employment (required before starting some positions).
  7. Possess strong interpersonal skills and the ability to collaborate effectively within a team.
