What are the responsibilities and job description for the Cancer Prevention Care Coordinator [Medical Assistant] position at CommUnityCare?

Overview:
In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments, the medical assistant will serve as a care coordinator for a cancer prevention program, funded by the Cancer Prevention Research Institute of Texas (CPRIT), to increase colorectal cancer-screening rates among the CommUnityCare patient population. This individual is responsible for supporting the goals of the program by preparing the screening kits and mailing them to eligible patients as well as communicating with care teams as related to the program.
This is a three (3) year grant funded position and will be a co-hire with the University of Texas at Austin Dell Medical School.
Responsibilities:
Essential Duties:
  • Prepare materials for and assemble packets for mailing colorectal cancer screening kits directly to eligible patients.
  • Receive returned notifications from patients and results from an external laboratory, and direct this information to the appropriate destination (patient, provider, database, etc.)
  • Complete data entry and assist analyst with database management.
  • Update CommUnityCare patient records when appropriate and locate outside records.
  • Act as a resource for internal and external questions related to the program and communicate with the project team at the University of Texas at Austin Dell Medical School.
  • Collaborate with the Patient Navigator to meet CPRIT Research Grant objectives, and meet regularly with the Program Coordinator from the University of Texas at Austin Dell Medical School.
  • Participate in the identification of patient needs for referral to resources that facilitate the continuity of care.
  • Perform other duties as assigned related to the CPRIT Research Grant in collaboration with the University of Texas at Austin Dell Medical School.
  • Complete all direct and indirect care documentation in timely manner and ensure patient records are current and complete.
  • Attend patient care (unit) huddles and periodic informational (clinic) huddles, meetings, patient conferences, or planning sessions to share information related to colorectal cancer screening status. These may be related to quality assurance, patient care and other related topics within the clinic.
  • Participate in quality strategies to evaluate compliance with evidence-based guidelines, standards and to identify opportunities to improve patient outcomes.
  • Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.
  • Develop and maintain favorable internal relationships, partnerships with co-workers, including clinical managers, clinical support staff, providers and business office staff.
  • Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families.
  • Collaborate with all members of the care team in providing patient-centered care.
  • Collaborate and communicate with the University of Texas Dell Medical School Department of Medicine Program Coordinator.

  • Excellent verbal and written communication skills.
  • Demonstrated knowledge of the four steps in systematic process, which include assessment, planning, implementation, and evaluation.
  • Demonstrated success in obtaining patient history using a structured assessment tool.
  • Demonstrated ability to identify common actual and potential social needs of patients.
  • Demonstrated familiarity with tools, technology, and systems typically found within most health care environments (i.e. personal computer skills, spreadsheets, word processing, patient records systems, EMR systems, etc.).
Qualifications:
MINIMUM EDUCATION:
  • Graduation from high school or the equivalent
  • Medical Assistant Diploma

MINIMUM EXPERIENCE:
Completion of a medical assistant externship

REQUIRED CERTIFICATIONS/LICENSURE:
Basic Life Support (BLS) certification obtained through approved American Heart Association or American Red Cross. Will be required to maintain a current BLS certification during employment.
