Responsibilities:

- Communicate Plan of Care with Patients or their families/caregivers. Provides high quality plans of care for patients with acute and chronic illnesses. Ensure accuracy of medical documentation to reflect the reason for the patient visit, history of illness, review of symptoms,physical examination and plan of treatment for each condition
Complete and electronically sign all progress notes daily, Prevent diseases by providing patient education concerning diet, appropriate disease preventive screenings and lifestyle modification,including good hygiene practices and exercise
- Valid Advanced Practice Registered Nurse (APRN) license in the state of {location}
- Master's degree in nursing or related field
- Certification in a specialized area of practice (e.g., Family Nurse Practitioner, Psychiatric-Mental Health Nurse Practitioner)
- Strong clinical knowledge and skills in the relevant specialty area(s)
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively within a healthcare team
- Proficiency in electronic medical records (EMR) systems
- Prior experience in a similar role is preferred, Candidate must be able to speak English and creole.
