JOB SUMMARY:
The CCBHC Behavioral Health Case Manager is a member of the behavioral health/family health services team(s). The teams are composed of multidisciplinary professionals including; Behavioral Health Clinicians, Behavioral Health Peers, Primary Care Providers, Psychiatric Providers, Nurses, Medical Assistants and Social Workers. The CCBHC Behavioral Health Case Manager works closely with Peers Support Specialists, primarily in the community, to support the needs of behavioral health clients and patients. The CCBHC Behavioral Health Case Manager helps clients increase their penetration into the community health system of care and access to quality behavioral care. This is done through outreach and engagement, professional relationship building, and assistance with coordination of care plans.

This position requires prolonged sitting, some bending, stooping, and stretching. It requires eye-hand coordination, and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, and other office equipment. It also requires a normal range of hearing and eyesight to record, prepare, and communicate appropriate reports.
In addition, this position requires travel between sites and the ability to interact with others in both small and large group settings. At times, this position may include periods of extended physical activity, such as walking/standing at community events or to perform outreach to clients.
