We are hiring a PRN Compassionate Licensed Medical Social Worker (LMSW or LBSW) to provide hospice social services in and around the area! If you are looking for a career with a purpose and a supportive team culture, then this is the position for you!
Our Licensed Medical Social Workers (LMSW) are responsible for the delivery of various social work services to hospice patients and their families.
Our Social Workers (LMSW) provide social service counseling and guidance to facilitate the maximum functioning and coping capacity of each patient and caregiver / family member.

Responsibilities :
- Assesses the psychosocial status of patients and families / caregivers related to the patient's terminal illness and environment and communicates findings to the RN Case Manager and other members of staff including the interdisciplinary group.
- Assess the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources, including funeral arrangements if needed.
- Supervises the implementation of an individualized social work plan of care and notified the Bereavement Coordinator of bereavement assessment needs.
- Counsels patients and family / caregivers as needed regarding stress and other identified coping difficulties.
Qualifications :
- Masters of Social Work from an accredited school of social work or Bachelor of Social Work.
- Holds current unencumbered license of Social Work in Texas.
- Minimum one (1) year experience as a social worker in long-term care or medical-surgical / acute care setting.
- One (1) year of hospice experience is preferred but not required.
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be computer proficient in typing and various programs, including a background in EMR.
- Must have the ability to handle the stress associated with ministering to patients / residents and their loved ones who experience pain, suffering, death, and grief.
- Possess and maintains current CPR certification.
