Case Manager –
We are looking for an experienced Case Manager that is responsible for providing comprehensive care management services to an ongoing caseload of skilled residents in our Skilled Nursing Facility. This position is responsible for ensuring that the residents receive quality and timely care that is in accordance with their individualized care plan.
This is a fast-paced role, and requires an individual that is highly-organized, care-focused and has excellent customer service skills with patients, families and our partners/vendors.
Essential Duties and Responsibilities:
- Develop, implement and monitor individualized care plans for residents in accordance with their medical and psychosocial needs.
- Coordinate and monitor the delivery of services to residents, including nursing, social services, therapy, dietary, and other services.
- Have at least 2 years’ experience in case management at a skilled nursing facility in Orange County working with major insurance groups.
- Ensure that resident care is provided in accordance with the facility’s policies and procedures, as well as all applicable state and federal regulations.
- Monitor and document the progress of resident care and submit to their Insurance case managers on a weekly basis.
- Serve as a liaison between the facility and other healthcare providers and community resources.
- Send clinical and rehab notes to case managers on a regular basis to assess patient progress.
- Assist in the development and implementation of discharge plans including all home health/DME.
- Participate in interdisciplinary team meetings and other meetings as required.
- Provide education and counseling to residents and their families about their care plan and options.
- Maintain accurate and timely documentation of all resident care activities.
- Maintain a current knowledge of applicable regulations and standards of care.
- Participate in quality assurance activities.
- Perform other duties as assigned.
Qualifications:
- Licensed Vocational Nurse
- At least two years of experience in a healthcare setting
- Knowledge of applicable regulations and standards of care in Orange County
- Excellent interpersonal, communication, and organizational skills
- Ability to work independently and as part of a team
