Summary:
Responsible for planning and administering social services programs. Supervises facility social workers. Assists in development of policies regarding participation in facility planning for health and welfare services
Roles and Responsibilities:
Plans, develops, organizes, implements, evaluates, and directs the social service programs of the facility
Keeps abreast of current federal and state regulations, as well as professional standards, and makes recommendations on changes in policies and procedures to the Administrator
Reviews department policies and procedures, at least annually, and participate in making recommended changes
Participates in discharge planning, development and implementation of social care plans and resident assessment
Interviews residents/families as necessary in a private setting
Refers resident/families to appropriate social service agencies when the facility does not provide the services
Provides information to residents/families as to Medicare/Medicaid, and other financial assistance programs available to the residents
Provides consultation to members of our staff, community agencies, etc., In efforts to solve the needs and problems of the resident through the development of social service programs
Assumes the authority, responsibility, and accountability of directing department
Maintains quality working relationship with the medical profession and other health related facilities and organizations
Reviews and evaluates the department's work force and makes recommendations to the Administrator
Coordinates social service activities with other departments as necessary
Works with the facility's consultants as necessary and implements recommended changes as required
Makes written and oral reports/recommendations to the Administrator concerning the operation of the social service department
Assist in standardizing the methods in which work will be accomplished
Interprets the department's policies and procedures to employees, residents, visitors, government agencies, etc
Reviews and develops a plan of correction for social service deficiencies noted during survey inspections
Participates in surveys process conducted by authorized government agencies
Interviews residents, or family members, as necessary to obtain social history
Assures social service progress notes are informative and descriptive of the services provide and of the resident's response to the service
Involves the residents/families in the planning objectives and goals for the resident
Organizes and facilities family council meetings
Assists with coordination of room moves
Assists with completion of transportation requests
Other duties as assigned
Education/Experience:
Undergraduate degree in Social Work or related field
Graduate degree is preferred
Minimum two (2) years' experience in a supervisory capacity in a hospital, skilled nursing care facility, or other related medical facility required
