Full job description
A growing hospice agency based in Dallas, TX, is seeking a motivated, goal-oriented Part-time Medical Social Worker to contribute to serve our patients in Dallas, Collin and Tarrant Counties.
What You Must Have:
Education: Master's Degree in Social Work from a State accredited program
Licensure: Current State license required in accordance with State regulations
Valid driver’s license and car insurance
Experience: One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred
Skills: Working knowledge of community resources. Good interpersonal skills.
Job Summary:
Medial social services will be provided by a qualified social worker, under the direction of a physician. Social work services will be based on the patient’s psychosocial assessment and the patient and family’s needs. The social worker will participate in the coordination of care with the Interdisciplinary Group.
What You Will Do: :
Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
Facilitate community resources based on patient/family needs.
