Connect Plus Therapy is looking to expand its services and is looking for a talented Case Manager who is a team player! The Case Manager must possess awesome communication skills as well as the ability to understand and carry out general instructions in standard situations. We are looking for outgoing, reliable individuals our families can count on! Connect Plus Therapy currently provides ABA therapy to children and would like to be able to support our clients and their family's even more.
Job Summary:
The Case Manager position is an administrative position that will be supporting Connect Plus Therapy’s clients by ensuring that their sessions are scheduled and will stay in constant communication to ensure the client gets the best services possible. This person must possess exceptional verbal and written communication skills. This position reports directly to the Administrative Director. In this role the Case Manager will also be responsible to maintain all client data and will be the clients go to person for support. This position will work 40 hours per week in an office setting.
Major Duties:
- Building relationships with the families of Connect Plus Therapy
- Scheduling meetings with parents to ensure that schedules are discussed.
- Assigning Behavior Technicians clients for services
- Data entry for client demographics and intake information.
- Support the clinical team by being a mediator between the staff and the client.
- Maintaining regular communication with the clients to ensure treatment is beneficial for the family and the client.
- All other duties as assigned
Skills and Competencies
- Proficient in the use of various computer programs including Central Reach, Microsoft Office, Microsoft Excel, and Google Suites.
- Confident communication skills for communications between the team
- Excellent written communication skills for professional emails and various reporting methods
- Excellent time management skills
- Ability to empathize with the families
- Self motivated and well organized
Physical Requirements:
- Ability to write, type, and operate a computer and telephone for hours at a time
- Ability to sit and stand for several hours at a time
- Ability to lift up to 40 pounds
