What are the responsibilities and job description for the Case Manager position at Connecticut Junior Republic?
GENERAL STATEMENT OF DUTIES AND RESPONSIBILITIES:
The Case Manager provides comprehensive case management and support for the adolescents/young adults participating in a transitional living program which provides participants with the skills and abilities to transition to independent living. This position is responsible for intake assessment/screening, and development, implementation and management of the youth's service plan with the goal of a successful transition upon completion of the program.
MINIMUM QUALIFICATIONS/REQUIREMENTS:
- Bachelor's degree with 1 year of relevant experience or, 5 years of experience in case management
- Experience working with a young adult or juvenile justice population
- Ability to Maintain a Flexible Schedule
- Bilingual (English/Spanish) a plus
- Must have a valid driver’s license
