Family Care Associate Director/Case Management Coordinator
Primary Purpose:Works to support the Director of Family Care based on the vision and goals of the Decade of Dreams Strategic Plan while working collaboratively in a team environment to provide family-centered, Christian valued, social work services to clients on a daily basis.
Essential Functions:
Co-develops a Comprehensive Care Plan with each family and coordinates services to each family
Leads and directs in family's plan of service to ensure the family receives maximum benefits of services
Provides mentoring and Christian valued counseling to client families within the scope of the professional's code of ethics
Maintains appropriate and required written documentation for each family's case record, as well as periodic reporting to administration, team, and other agencies that might be involved
Functions as an agency team member working with others to provide quality services to the clients and community outreach
Function as part of the crisis intervention for the family care program including on-call
Functions as a TEAM member within Family Care and Connie Maxwell
Support CMCM vision, mission, and values
Work towards achieving yearly "Gold Star" list
Other duties as assigned by Supervisor
Qualifications:
Bachelor's Degree from an accredited college/university in one of the Human Services areas is required; Master's Degree is preferred
Licensed as MSW or LPC is preferred
Seven or more years of experience in some area of family welfare, mental health, or social service field
Valid driver's license with insurability is required
Ability to help clients consider options and make choices and decisions
Ability to communicate and work well within a team
Ability to lead and to work effectively with various staff and work styles
Crisis intervention knowledge/skills
Ability to communicate; supportive individual and group counseling skills, planning, and casework coordination skills
Able to present in group settings
Ability to assess limited information and make decisions regarding intake and discharge
Knowledge of and ability to work with community sources
Sensitive to traumatic effects of separation, placement, and understanding of the grief process
Knowledge of developmental tasks and the implications of trauma and pain based behavior for families in care and the skills to take steps necessary to address the issues

Work Conditions:
Sitting, standing, and/or driving for extended periods of time
Dexterity of hand and fingers to operate a computer keyboard, mouse, and to handle other computer components
Ability to work a flexible schedule including after hours and on-call responsibilities, weekends and evenings based on ministry needs and events
Ability to work under stress during peak workload periods
