**Job Summary:**

We are seeking a dedicated CBRS /Case Manager to join our team. Worker will be responsible for providing skills buildiing and support to individuals with a mental illness diagnosis or dual diagnosis to enable them to live as independently as possible with supportive services.

**Duties:**

- Conduct assessments to determine client needs and develop individualized care plans

- Coordinate services and resources to meet the client's goals and objectives

- Provide medication administration assistance as needed

- Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care

- Maintain accurate records and documentation of client progress

- Facilitate communication between clients, caregivers, and service providers

**Skills:**

- Proficiency in caregiving techniques and practices

- Strong interpersonal and communication skills

- Ability to collect and analyze data for monitoring client outcomes
