What are the responsibilities and job description for the LPN-Health Services Coordinator position at Consumer Support Services, Inc.?
Consumer Support Services is currently hiring for the position of LPN for our 8 bed ICF/IID Facility:
The following job duties are required:
1. Assure Consumer’s Rights are observed.
2.Responsible for daily oversight of a maximum of 6 individuals in the ICF/IID setting.
3.Oversee and/or coordinate medical care and treatment to the consumers as required, per doctor’s orders and the IHP.
4.Provide professional nursing services to each consumer; performing basic therapeutic and preventative nursing measures according to established protocols.
5.Maintain medical records and insure compliance with all state, local and federal guidelines.
6.Serve as a liaison with consumers, physicians, pharmacists, agencies and significant others; including attending physician appointments with the individual.
7.Meet with applicable representatives from any regulatory agency regarding all medical aspects of the company.
8.Provide in-service training to staff in compliance with regulatory agency standards as assigned.
9.Provide oversight for medication administration as assigned and assure that all medications are administered, dispensed, stored and disposed of by the staff in compliance with regulatory standards and company policy.
10.Participate in and oversee a preventative program for infection control.
11.Monitor the consumer for adverse reactions to medications and report to the physician.
12.Purchase and/or requisition needed medical supplies and equipment to assure an adequate supply is available.
13.Participate in, and provide input at team meetings to assure optimum development and health for each consumer. Generate all IHP forms for all consumers while attending the IHP meeting.
14.Complete all assessments as required.
15.Coordinate services of contractual personnel related to medical services as assigned (i.e. dietician, pharmacy, lab, medical suppliers).
16.Be on-call as assigned.
17.Participates in quality assurance reviews in all areas of consumer care. 18.Participate in physician rounds.
19.Call to hospital to check on Consumers that are hospitalized.
20.Monitor Monthly Flow Sheets, i.e. bowel Movement, Menstrual Flow sheet, I and O, etc.
21.Communicate consumer health care situations to the County Director, Program Director, Program Coordinator, Supervisor, QMRP, Adult Day Service, and County Board as necessary.
22.Provide delegating nurse services as assigned.
23.Act as a resource for clarification and interpretation of health care issues.
24. Adhere to all safety standards set forth per regulatory agencies and company policy.
25.Perform other related duties as assigned.
26. Travel to Dayton one day a week.
27. Assist with training classes for CPR/1st Aid and Delegated Nursing - recert not initial class.
Job Type: Full-time
Pay: $23.00 - $25.00 per hour
