Clinical Records Reviewer

General Statement of Duties:

Working in Continuous Quality Improvement, assisting with quality assurance and compliance with external and Department standards regarding case records via chart reviews and the use of reporting tools within the electronic health record. Assists and facilitates promotion of effective and efficient delivery of services; does so via review and analysis of data related to chart reviews. Facilitates completion of Root Cause Analyses. Facilitates external audits. Conducts investigations as needed. Potential for Hybrid schedule exists.

Specific Duties:

• Perform routine chart reviews of clinical records.
• Perform non-routine chart reviews to address concerns and inquiries regarding the clinical documentation of specific issues.
• Facilitate and perform root cause analyses of sentinel events involving individuals participating in behavioral health or development services programs.
• Conduct reviews of clinical documentation related to updates and changes in external and internal documentation requirements. (e.g., regulatory, insurance related)
• Conduct reviews of clinical documentation related to the implementation of new services to ensure compliance with external and internal standards.
• Use electronic record reporting tools and distribute information to staff members and supervisors to help ensure compliance with documentation standards
• Assist in the development and updating of chart review tools to ensure that they are consistent with standards and requirements
• Analyze information obtained related to chart reviews; write summary reports and present information to members of Behavioral Health and Developmental Services Leadership.
• Assist with organizing information to respond to audits.
• Assist with investigations as needed.

Specific Credentials (Licensure/Certifications): Licensure as a mental health professional (LMHP, QMHP) or RN preferred.

Minimum Qualifications (Education/experience/etc.): Masters Degree

Knowledge of mental health, substance abuse, and intellectual disability services for adults and children; experience working with computers, computer reports, and spreadsheets; experience with the monitoring and utilization of data.

A combination of education (at least a bachelor’s degree) and experience equivalent to nine (9) years in fields utilizing the knowledge, skills, and abilities listed above and associated with such positions as clinician, counselor, therapist, social worker, psychologist, or psychiatric nurse.

Job Types: Full-time, Contract

Pay: $32.18 per hour
