HEALTH EQUITY COORDINATOR (FOOD SECURITY)
DEPARTMENT: EQUITY & INCLUSION
LOCATION: JOHN STROGER HOSPITAL
This position is exempt from Career Service under the CCH Personnel Rules.
Job Summary
Under supervision, the Health Equity Coordinator supports efforts related to identifying, addressing, and connecting Cook County Health patients with resources to address social determinants of health/social risk factors, including food, housing, transportation, social and economic development, along with other domains as assigned. The Health Equity Coordinator helps patients connect with resources that reduce barriers that interfere with the patient's ability to achieve their health goals.
Typical Duties
Coordinates with internal and external stakeholders to identify, connect, create, and implement programs to address social determinants of health/social risk factors.
Documents all activities according to policies and procedures.
Assists with the preparation of grant and program reports.
Coordinates education and conducts outreach to patients on resources that address social determinants/social risk factors.
Attends and participates in relevant internal and external meetings.
Completes all education activities/training as required by state or accreditation standards.
Attends training and receives certification to support patient education, e.g., diabetic education.
Protects Protected Health Information (PHI) and complies with Cook County Health PrivacyPolicy.
Travels to CCH works sites or throughout Cook County, when needed.
Performs other duties as assigned.
Minimum Qualifications
High School Diploma or GED
One (1) year of experience working with individuals or programs in a community-based setting
Valid driver's license and mandatory vehicle insurance where required
Ability to lift to 50 pounds as required
Proficiency using Microsoft Office applications

Physical and Environmental Demands
This position is functioning within a healthcare environment. The incumbent is responsible for adherence to all hospital and department specific safety requirements. This includes but is not limited to the following policies and procedures: complying with Personal Protective Equipment requirements, hand washing and sanitizing practices, complying with department specific engineering and work practice controls and any other work area safety precautions as specified by hospital wide policy and departmental procedures.
