Who We Need  

The MACT Rehabilitation Specialist plays an important role on the Medical Assertive Community Treatment (MACT) team by helping members obtain and maintain involvement in employment and/or community-based activities. Works as member of a clinical team to assess member needs, participates in the development of individual service plans, and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Serves as the MACT team’s specialist for rehabilitation services by collaborating with other team members in the assessing and delivery of rehabilitation skills training and meaningful community activity as specified in the individual consumer’s service plan
MINIMUM QUALIFICATIONS:
  • High school diploma or GED, and at least four (4) years of full-time behavioral health work experience.
  • Bachelor’s Degree in a related field OR a bachelor’s degree in a non-related field with one (1) year experience preferred.
  • Must have one (1) year of training or experience in vocational rehabilitation and support. Prior experience may include job development, job coaching, or supported employment highly preferred.
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
  • Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
  • TB Test clearance required at the time of hire.

JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO) 

  • Works as part of a clinical team to facilitate recovery and inspire hope for members while supporting their achievement of service plan goals. Provides ongoing assessment and feedback as to their progress in relation to their service plan.
  • Meets with members in the community in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed.
  • Works with member’s Prescribing Clinician, Nurse, and other members of the clinical team to ensure the timely development and implementation of the individual’s service plan (including all necessary assessments) and their crisis plan.
  • Coordinates with all parties involved in member’s treatment according to policy and procedure and clinical protocols, including but not limited to attending inter and intra-agency staffing, communication by telephone and letter, service/support delivery, etc., as indicated on the member’s r’s service plan.
  • Assists members and families in accessing additional needed services and obtaining entitlements, including identification of funding services.
  • Responsible for transporting members to necessary services, as outlined in their service plan, as needed.
  • Assumes “primary contact” responsibility for a percentage of MACT members to: ensure maintenance of accurate, thorough, and current documentation in the medical record (in accordance with policies, procedures, and clinical protocols) for those specific individuals; keep weekly client schedule for those individuals updates, and act as first contact for those consumers and their families.
  • Completes all clinical documentation in a timely manner in accordance with policies, procedures, and clinical protocols and meets/exceeds member engagement goals as defined by Copa Health.
  • Participates in daily team meetings as well as any additional individual treatment teams or other team communication meetings as required.
  • Works in collaboration with team members to ensure compliance with MACT fidelity measures.
  • Collaborates with the MACT members to complete the contractually required rehab/employment assessments and then addresses the needs identified by that assessment for each MACT member.
  • Participates in the service plan goal development for each member as it related to their rehabilitation goals and the interventions they will need.
  • Provides and oversees the provision of any direct skills training(s) indicated as needed by the individual MACT members.
  • Works in close collaboration with the Independent Living Skills Specialist, Housing Specialist, Employment Specialist, and the assigned RSA Vocational Rehabilitation Counselor to ensure coordination of services.
  • Provide and track detailed information on all employment related engagement and caseload.
  • Utilize psychiatric rehabilitation best practices including evidence-based employment practices to assess participant’s needs emphasizing a Copa Health Work First philosophy for appropriate individuals to reach their desired outcome.
  • Educates and assists members in identifying the resources needed to obtain and maintain employment, including groups and workshops at respective outpatient locations.
  • Works closely with the Employment Specialist in providing limited job coaching virtually, within the health home, and in the community.
  • To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.
