What are the responsibilities and job description for the Home Health Aide position at Cornerstone Caregiving?
Duties:
Client's schedule is Monday, Wednesday, Friday 8a-5p and Tuesday/Thursday 8a-noon
- Provide direct patient care in a home setting, following a care plan provided.
- Assist patients with activities of daily living, such as bathing, dressing, and grooming.
- Administer medications as prescribed by healthcare professionals.
- Assist with mobility and transfers, using proper lifting techniques and assistive devices.
- Prepare meals and assist with feeding, ensuring dietary restrictions and preferences are followed.
- Perform light housekeeping tasks to maintain a clean and safe environment for the patient.
- Provide companionship and emotional support to patients and their families.
- Document patient care activities accurately and timely in medical records.
Qualifications:
- Previous experience in home care or a related field is preferred
- Knowledge of hospice care principles and practices is a plus.
- Ability to cook nutritious meals for patients with specific dietary needs.
- Excellent interpersonal skills to communicate effectively with patients, families, and healthcare professionals.
- Ability to provide compassionate patient care while maintaining professionalism.
- Physical ability to lift and move patients safely.
- MUST be willing to commute to Lake County for work.
