Position Overview: We are seeking a dedicated and organized individual to join our team as a Hospice Department Scheduler and Administrator. This pivotal role involves overseeing both non-clinical and clinical scheduling duties and providing essential administrative support to ensure the efficient operation of our Hospice Department. Responsibilities:

  • Collaborate closely with the Hospice Director and Health Information RN to coordinate hospice services effectively.
  • Assist the Hospice Department with clerical and clinical tasks, providing day-to-day support and assisting with special projects.
  • Handle phone calls, faxing, scanning, copying, and emailing of clinical information related to patient referrals, transfers, discharges, or changes in care.
  • Coordinate the ordering and delivery of Durable Medical Equipment (DME) as necessary.
  • Communicate urgent health information/triage calls to field staff promptly and appropriately.
  • Take the lead in hospice triage, referral coordination, and timely communication with the hospice team.
  • Manage scheduling changes for hospice visits and support staff with adjustments as needed.
  • Maintain schedules for nursing and Licensed Nursing Assistants (LNAs) and assist other departments when required.
  • Directly supervise and manage hospice LNAs, conducting regular meetings and ensuring follow-up on action items.
  • Make schedule changes, notify staff and patients/families, considering patient needs and caseloads.
  • Run and manage all scheduling reports daily to perform the job effectively.
  • Notify the Hospice Director of scheduling conflicts and proactively address them.

Requirements:

  • Required to be either a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • Previous experience in a healthcare setting, preferably in Hospice or Palliative Care.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and other office equipment/tools.
  • Excellent organizational and time-management skills with the ability to prioritize tasks effectively.
  • Strong communication skills, both written and verbal, with the ability to interact professionally with patients, families, and staff.
  • Ability to adapt to changes in the work environment and manage competing demands.
  • Commitment to maintaining compliance with regulatory rules and regulations, policies, and procedures related to the role.
