We are seeking a WV licensed social worker with a Bachelor’s degree from an accredited college or university to assist with the coordination of care of residents of a long-term care facility. Familiarity with current federal, state & local regulations is beneficial, including short-term rehab discharge planning. Two years of related work experience preferred. Good written & verbal communication skills. Must be organized and goal oriented. Candidate should possess excellent people skills. EOE
