RN Care Coordinator
This position is responsible for coordinating team-based care to provide health services to individuals, through effective partnerships with patients, their families/caregivers, internal and external resources and their providers. This position will also serve as the population health nurse for both our ACO and Medicaid Transformation Projects. The RN Care Coordinator supports the clinic overall objectives by providing a coordinated, strategic approach to detect early and manage effectively the chronically ill patient population through implementing an effective internal tracking system for identified patients. This position will also work as a Clinic Nurse and Clinic Charge RN if needed.
This position is eligible for the following bonus details:
- INTERNAL SELECTED CANDIDATE- Eligible for $3,000 retention bonus after the end of first year and additional $3,000 retention bonus at the end of second year.
- EXTERNAL SELECTED CANDIDATE- Eligible for $3,000 sign on bonus after 90-day evaluation and additional $3,000 retention bonus at the end of second year

Salary Range:
The wage scale for this position has been finalized for the facility including 26 steps and the selected candidate will be placed on the wage scale depending on years of experience.
