SPECIFIC RESPONSIBILITIES: 

  1. Coordinate all medical-related aspects of the program, including assisting members/clients in taking/obtaining medications, educate them on medications and possible side effects, order/maintain medical supplies, secure necessary lab work, and communicate with outpatient clients concerning medications, and inform case managers of changes in medical needs of members in the CSS program.
  2. Assist medical professionals, including arranging appointments, securing files, assisting in obtaining pertinent data concerning members/clients, and transcribe all medical related reports.  May assist Medical Records Manager with Utilization and Review.
  3. Acts as first contact in client/staff medical situations/emergencies as required by Clinic Health & Safety Policies.
  4. Responsible for providing case management services as appropriate.
  5. Perform other duties as assigned by the Program Directors.

 JOB RELATIONSHIP: Responsible To:  Community Support Services Director when working in CSS Program & Outpatient Services Director when working in outpatient clinic.

 JOB QUALIFICATIONS:  LPTN, or LPN,  experience in working with the mentally ill preferred.
