This is a full-time position with a required EOW and EOH rotation.

Country Manor's Rapid Recovery & Aquatic Center is a sub-acute rehabilitation facility. The state-of-the-art, separate entrance building, boasts 64 private suites with private baths, lounges, salon & spa, and an inpatient and outpatient rehabilitation gym, complete with a HydroWorx Warm Water Therapy pool. The environment is designed intentionally to be warm and inviting, creating an optimal space for patient recovery. Visitors and staff also enjoy the serene surroundings.

The Team Leader is responsible for providing direct care to the residents and serving the needs of residents in a manner conducive to their safety and comfort. Must possess leadership qualities, have excellent attendance record, and a positive teambuilding attitude. The Team Leader must abide by the philosophy, procedures and policies of Country Manor Health Care & Rehab Center.

Responsibilities:

• Maintain quality of life for residents through programming and following individual care plans to accommodate needs and customary routines.
• Must assume responsibility caring for and serving the needs of the residents.
• Must be aware of areas that make residents subject to abuse and prevent such abuse.
• Must be aware of safety issues and strive to complete all responsibilities in a safe manner.
• Must be able to identify safety hazards and emergency situations and initiate corrective actions.
• Must communicate resident concerns to supervisor.
• Actively participate in evaluation process of NA/R team members.
• Must be willing to mentor and nurture new team members.
• Oversee that cares, assignments and documentation are completed.
• Create a positive atmosphere for team building.
• Role model the teamwork concept.
• Maintain skills according to standards of practice.
• Must attend care center meetings per Director of Nursing request. Provide client care as required under the state licensure
• Provide basic patient care and treatment
• Collaborate with registered nurses
• Educate and update patients on medical treatments
• Communication and interactions with clients, families, and other health team members are an essential component of this position
• Must provide all CNA Direct Care Responsibilities

Qualifications Required:

• 18 years of age.
• Certified in Minnesota as a Nursing Assistant
• CPR Certified
• 6 months experience as a certified nursing assistant
• Ability to work with peers, supervisors and residents in a cooperative, considerate and helpful manner.
• Ability to effectively communicate with residents, families and personnel.
• Ability to handle physical and/or verbal abuse from residents in an appropriate professional manner.
• Ability to defuse situation/conflicts through a resolution process.
• Possess leadership qualities.
• Willing to learn caregiver techniques and problem-solving skills.
• Preceptor experience
• Possess excellent organizational skills and time management skills
• Ability to maintain emotional stability in emergency situations
