Job Posting for Admissions Liaison at County of Chester
The Pocopson Home Admissions Liaison, in coordination with the Admissions Director and Social Services Director, is responsible for the efficient transition of long term and short term admissions and discharges. This position will also assist social services with evaluations and assessments.
A criminal background check is required.
Assist new admission and family through the admission process until admission is complete.
Provide emotional support and a positive approach to family and resident to ensure a smooth transition.
Provide essential feedback to the Admissions Director of any potential problems discovered during the admission process.
Complete pre-admission on-site field visits as needed, ensure that an accurate and timely report is completed reflecting the resident's needs, and any potential issues that are observed.
Provide tours to potential residents and their families as needed.
Distribute handouts regarding Pocopson Home policies and procedures, and explain policies and procedures to new admission and their families.
Handle admission referrals via phone, fax, email and personal contact.
Interface with staff from Chester County Department of Aging Services, Chester County Assistance Office and case managers from hospitals and other health care facilities and agencies.
Prepare marketing materials and market the facility to potential admissions.
Coordinate admission day activities with social service, accounting and nursing departments.
Inspect new resident's room, send clothes to laundry for labeling, label resident's personal items and set up resident's room with their personal items on the day of admission.
Present a positive image of Pocopson Home to external community.
Be experienced in case management and have the ability to document as needed in the medical record.
Must be a people-person and be available for flexible hours.
Perform other duties, tasks and special projects, as required.
Qualifications / Requirements:
Minimum of one and one-half years of experience working in a human services/admission environment.
Excellent verbal and written communication skills.
Strong marketing skills.

Ability to work independently as well as part of a team.
Strong professional ethics.
Excellent interpersonal skills.
Strong organizational and time management skills.
Ability to maintain confidential information and handle confidential matters, HIPPA regulations.
Familiar with State and Federal rules and regulations.
A valid driver's license is required.
Valid automobile insurance.
Must use own personal vehicle for on-site field visits.
Preferred Skills, Knowledge & Experience:
Experience in Community Health, Social Work, Marketing or a related field.
Ability to follow office protocols.
Able to use common sense understanding to carry out written or verbal instructions.
Ability to handle and resolve recurring problems.
Ability to establish priorities (remain focused on daily operations).
Able to carry tasks to completion.
Basic computer knowledge.
Ability to work as part of a team.
