POSITION: AGING CARE MANAGER I

Full-Time; 80 hours per pay

DEPARTMENT: OLDER ADULT SERVICES

PAY GRADE: HUMAN SERVICES NON-EXEMPT HOURLY PAY GRADE ONE

$33,009.60/yr $750 SIGN-ON BONUSES IF FILLED EXTERNALLY

OVERALL OBJECTIVE OF JOB

To perform assessment, provide service coordination, and develop care plans for older adult individuals and their families in order to maintain a quality, independent living lifestyle.

SUPERVISION RECEIVED

Receives occasional instruction and some supervision from Supervisor in regard to daily work duties.

ESSENTIAL FUNCTIONS OF THE JOB

1. Assesses individuals’ overall situation, including abilities, disabilities and needed resources; completes comprehensive needs assessments.

2. Develops care plans with individuals and family to meet the needs of the individual.

3. Assists older adults with completion of applications and forms.

4. Coordinates needed services for individuals, and interacts with County and outside agencies as needed.

5. Adheres to applicable rules and regulations of the CHC Medicaid Waiver program.

6. Interprets federal and state regulations and/or county policies and procedures to individuals, groups, and agencies.

7. Provides information and referral services for older adults.

8. Acts as individual’s advocate in appropriate matters.

9. Travels as necessary to meet and/or assist individuals to obtain or arrange needed services.

10. Prepares case notes, re-assessments and other related studies and paperwork as required by agency policy or regulation.

11. Maintains proper documentation and files and prepares required reports in timely manner.

13. Monitors changes in individuals’ conditions and recommends appropriate changes in services to supervisor and/or medical personnel.

14. Enters individual data in the SAMS database for maintenance of records; searches computer database for individual information as needed.

15. Operates computer and other office equipment as necessary to carry out essential job duties.

16. Bills individual’s insurance for eligible activities. Adheres to all regulations and guidelines pertaining to billing.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

1. Must be able to speak and understand the English language in an understandable manner in order to carry out essential functions of job.

2. Must possess good communication and interpersonal skills.

3. Must possess initiative and problem-solving skills in relation to social service work.

4. Must possess ability to function independently, have flexibility and the ability to work effectively with individuals, co-workers, and others.

5. Must possess the ability to maintain confidentiality in regard to individuals’ information and records.

6. Must possess a valid Pennsylvania Driver's License and/or daily access to reliable transportation throughout the county for completing comprehensive assessments in the homes of older people and be willing to travel in all sorts of weather conditions.

7. Must possess the ability to make independent decisions when circumstances warrant such action.

8. Responds to emergencies appropriately, including appropriately reporting abuse, abandonment, neglect or financial exploitation of an older adult related to the Older Adult Protective Services laws for investigation and possible emergency court hearings on emergency involuntary interventions.

9. Must possess knowledge of the principles and practices used in social service work, and the ability to apply these principles and practices within the job.

10. Must possess some knowledge of the medical concerns and related treatments and practices regarding the aging process.

11. Must possess some knowledge and ability to counsel elderly and to work in crisis situations.

12. Must possess knowledge of community resources and the ability to interact effectively with them on the individuals’ behalf.

13. Must possess ability to conduct complete assessments and to determine most appropriate, least restrictive level of care needed.

14. Must possess ability to deliver and evaluate appropriate services to individuals and to monitor the effectiveness of such services.

15. Must possess some technical knowledge of operating personal computers and related software, or ability to learn same and operate with reasonable accuracy and speed.

16. Must possess ability to understand and interpret laws, regulations, and policies in regard to older adult services’ programs and services.

QUALIFICATIONS

Associate’s degree majoring in the social welfare field and 3 years of professional casework or human service experience; or a bachelor’s degree majoring in social welfare and 1 year of professional casework or human service experience; or a bachelor’s degree in an unrelated field with at least 12 credits in social welfare and 2 years of professional casework or human service experience; or a bachelor’s degree and successful completion of an accredited casework intern program; or successful completion of 6-month probationary period as a Service Coordinator I.

Clearances:

  • Must work in conjunction with employer to apply for Act 34 PA State Police Criminal History, Act 151 PA Child Abuse and FBI clearances prior to employment. Clearances currently possessed that are less than 4 years old will be accepted until new clearances are available. All clearances must reflect acceptable results.
  • Must not appear on Preclusion Lists as defined by “Pennsylvania’s Medical Assistance Bulletin 99-11-05 Provider Screening of Employees and Contractors for Exclusion from Participating in Federal Healthcare Programs.”
  • Must successfully complete pre-employment drug screening.
