OVERALL OBJECTIVE OF JOB
To supervise the provision of services coordination to children, youth and families, people who are mentally disabled, people who are physically challenged and others to assist them in attaining a more satisfactory social, economic, emotional, or physical adjustment. Work involves responsibility for supervising and training a small group of professional county service coordinators. Work includes responsibility for assigning and reviewing the work of the service coordinators. Supervision may also be exercised over paraprofessional human service workers. Work is performed in accordance with established regulations, policies and procedures, but employees are expected to exercise considerable initiative and independent judgment in achieving objectives. Work is reviewed by a professional or administrative supervisor through conferences, reports, and results attained.
ESSENTIAL FUNCTIONS OF JOB
1. Supervises and supports service coordinators and assigns and monitors work within the department.
2. Reviews case records opened from the PIC Unit and makes assignments; conducts ongoing review of cases.
3. Assists coordinators in recommending and implementing various services to assist clients with difficulties.
4. Provides direct services to a designated caseload of clients, if necessary.
5. Provides daily instruction, support and direction to staff regarding any issues and ensures compliance with state and agency policies and procedures.
5. Suggests referrals to other agencies in order to assist in meeting the individual’s needs.
6. Conducts performance reviews of services coordinators and addresses any related personnel issues within department.
7. Oversees the individual’s transportation needs and scheduling arrangements.
8. Prepares reports, summaries and documentation as required and needed on individuals’ records and progress; utilizes computer frequently for record keeping/reporting requirements.
9. Oversees the development and implementation of service plans for individuals including use of community resources and natural supports.
10. May make necessary arrangements according to individuals’ needs including referrals/contacts to other community services/agencies to help fulfill the service plan/meet immediate needs.
11. Maintains accurate documentation and records on individuals and their progress and assures that assigned staff do the same.
12. Participates in training sessions for services coordinators.
13. Assists in development of services coordination’s plans and monitors progress of same.
14. Oversees unit Orders of Court to ensure that required information is relayed.
15. Assists and supervises preparation for court reviews and hearings, testifies as required at court hearings on the individual’s behalf, as necessary.
16. Attends training, in-services and meetings, as required.
17. Attends certification training as required under Act 151 of the Child Protective Services Law, Title 23 PA. CS & Chapter 6.
18. Reviews and analyzes case records to evaluate the quality-of-service coordination.
19. Reviews and evaluates the case assignments, services, actions taken, information provided, decisions made, service planning completed by service coordinators in the delivery of services both in oral consultation and review of record documentation.
20. Collects information regarding community resources and services.
21. Assists in the development of Agency policy and procedure.
22. Assists in program planning.
23. Holds regularly scheduled individual supervisory conferences.
24. Closely assists in the development of Internal Foster Care Resources.
Other Duties of the Job
1. Attends training, in-services and meetings as required.
2. Attends certification training as required.
3. Testifies at court hearings as required.
4. Performs other related work as required.
SUPERVISION RECEIVED
Receives regular instruction and supervision from the Program Director in regard to daily work duties and responsibilities.
SUPERVISION EXERCISED
Supervises an assigned group of service coordinators in an ongoing case coordination unit of Children and Youth.

8. Must possess the academic knowledge and some skill in promoting therapies and social service work and skills with children and youth and families and other individuals.
9. Must possess knowledge of child development and the ability to conduct assessments and investigations.
10. Must possess ability to provide structured and unstructured life skills instructions and guidance to individuals as needed.
11. Must possess the ability to express empathy and understanding to all individuals.
12. Must be able to interact effectively with children, youth, families, department staff, service providers, foster parents, counselors, attorneys, and the courts.
13. Must be able to work effectively with individuals involved in the criminal and/or juvenile justice system.
14. Must be able to rapidly establish rapport with individuals having behavioral health, substance abuse, or intellectual challenges.
15. Must have some knowledge of the judicial and court system as it relates to the child and family system.
16. Must have transportation available and a willingness to travel for work-related job duties.
QUALIFICATIONS
Bachelor’s degree in the social welfare field and 4 years of professional casework or human service experience; orBachelor’s degree in unrelated field that includes at least 12 credits in social welfare and 5 years of professional casework or human service experience; or Master’s degree in social welfare and 2 years of professional casework or human service experience; or 3 years of experience as a Venango County Service Coordinator.
Clearances:
· Must work in conjunction with employer to apply for Act 34 PA State Police Criminal History, Act 151 PA Child Abuse and FBI clearances prior to employment. Clearances currently possessed that are less than 4 years old will be accepted until new clearances are available. All clearances must reflect acceptable results.
· Must not appear on Preclusion Lists as defined by “Pennsylvania’s Medical Assistance Bulletin 99-11-05 Provider Screening of Employees and Contractors for Exclusion from Participating in Federal Healthcare Programs.”
· Must successfully complete pre-employment drug screening.
