Covenant Community Care, a faith based charitable non-profit Community Health Center serving the people of Metro Detroit. We are seeking a part-time Homeless Outreach Worker to join our Outreach team in Detroit. As a Federally Qualified Health Center, we offer integrated medical, dental, and behavioral health care to everyone, regardless of their ability to pay.
Homeless Outreach Worker will assist the care team and function as an educator, coordinator, advocate and facilitator of health and health services for individuals in the street medicine setting. Homeless Outreach Worker will work to improve the quality of patient care through ongoing communication and collaboration with the multidisciplinary care team for chronically ill homeless patients.
RESPONSIBILITIES:
- Coordinate and assist patients with transportation to and from healthcare appointments.
- Assist in patient registration as needed.
- Assist in the set-up, maintenance and cleanliness of homeless outreach vehicles, mobile clinics and remote sites in partnership with the Mobile Medical team and facilities department.
- Distribution of shoes, socks, boots, water, PPE, hygiene kits and care items from community partners in keeping with all FQHC guidelines and Covenant Community Care policies and procedures.
- Assist patients in referrals to resources for the reestablishment of identification, benefits, housing etc. Perform regular non-medical outreach establishing relationships of trust and building bridges to people experiencing homelessness.
- Provide transportation and non-medical support to providers and staff in the provision of medical care. Assist team in the set-up, cleaning and restocking of mobile clinic and remote sites.
- Documents all services delivered in a timely, accurate and prescribed manner, in compliance with the Covenant Community Care quality assurance plan.
QUALIFICATIONS
- Experienced in a patient advocacy role or in community service non-profit preferred
- Certified Peer Support Specialist or Certified Peer Recovery Coach preferred
- Prior experience working with homeless or at-risk populations preferred
- Great interpersonal and conversation skills
- Good driving record
- High school graduate

Salary : $27,300 - $34,600
