We are currently seeking a Support Broker to join our team of professionals in our Individual and Family Support Services.
Support Brokerage is a service available for individuals who self-direct some or all of their services. A Support Broker assists the individual initially with developing a self-directed budget and then with the day-to-day management of self-directed services. A Support Broker provides support and training to individuals and their families regarding the ongoing decisions and tasks associated with self-direction.
Hours of Employment
Full-Time. Days and hours of work are Monday through Friday during business hours. Evening work may be required as job duties demand. Flexible hours – days and evenings based on individual/family availability.
Essential Functions
- Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to children, adults, family members, program staff and community resources.
- Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
- Assists in the development and maintenance of a Circle of Support (also known as a planning team) and assists in directing planning meetings.
- Ensures that planning meetings occur at least four times a year and are face-to-face.
- Ensures completion of and regular updating of staff action plans for self-hired Community Habilitation and Supported Employment (SEMP) staff if applicable.
- Assists in the development of a comprehensive Self-Directed Budget that is consistent with the Individualized Service Plan (ISP)/Life Plan and works with the Individual/Family and their Circle of Support to ensure that all necessary safeguards are included and addressed in the ISP/Life Plan.
- Assists the Individual/Family to develop a comprehensive Person-Centered Plan.
- Provides education and training to the Individual/Family and Circle of Support in implementing the Self-Directed Budget according to Medicaid and New York State Standards.
- Monitors Self-Direction expenditures to ensure that spending does exceed the Self-Directed Budget.
- Assists the Individual/Family to properly document services according to Medicaid and New York State regulations and policy. This includes assisting the Individual/Family to review and submit required forms to the Fiscal Intermediary including but not limited to employee time sheets, the monthly summary note, mileage and expense reimbursement forms, and all other required documentation.
- Assists Individual/Family (as requested) in recruiting and hiring self-hired staff and completing service documentation.
- Provides training and support on hiring and supervising self-hired staff.
- Responsible for maintaining his/her training records that verify attendance at required trainings.
- Participates in conferences, workshops and in-service training as required or recommended to enhance job skills and personal growth and development.
Travel
Travel is primarily local during the business day, but employee may be expected to work at any of our partner agencies. Some out-of-the-area and overnight travel may be expected.

Requirements
- Bachelor’s Degree preferred
- Completion of an OPWDD approved curriculum Broker Training
- Experience preferred.
- Valid and clean New York State Driver License

Pay: $40,000.00 - $45,000.00 per year
